摘要
目的 研究虚拟现实(virtual reality,VR)手术模拟联合3D打印技术在耳科手术技能训练中的应用效果。方法 纳入2022年9月9—11日参加郑州大学第一附属医院颞骨解剖培训班的40名耳科医师作为研究对象。采用随机数字表法将学员分为2组,VR组(n=20)接受VR手术模拟训练(2次,每次2 h);VR联合3D打印组(n=20)先进行2 h VR手术模拟训练,随后使用3D打印颞骨模型进行2 h解剖操作。培训结束后,2组受试者均接受2 h传统颞骨解剖训练,完成乳突切除术。主要评估指标为基于任务的检查表(task-based checklist,TBC)得分,分别在VR手术模拟训练、解剖3D打印模型及颞骨解剖阶段进行对比分析。结果 在连续的训练中2组的TBC得分均高于前一次,差异有统计学意义(P<0.001),并且2组间得分差异有统计学意义(P<0.001)。在第2次训练及最终的颞骨解剖环节中VR联合3D打印组的TBC得分均高于VR组,差异有统计学意义(P<0.001)。结论 VR手术模拟训练与3D打印技术均可有效提升耳科医师手术操作能力,联合训练的表现更优,提示2种技术具有协同强化效应,为优化耳科手术培训路径提供了证据。
Objective To study the effectiveness of virtual reality(VR)surgery simulation combined with 3D printing technology in otologic surgical skills training.Methods A total of 40 otologists who participated in the temporal bone anatomy training course of the First Affiliated Hospital of th Zhengzhou University from September 9 to 11th,2022 were included as the research subjects.The trainees were divided into 2 groups by the random number table method.The VR group(n=20)was taught with VR surgery simulation training(twice,each time for 2 h).The VR combined with 3D printing group(n=20)was first trained in VR surgical simulation for 2 h and then underwent 2 h of anatomical manipulation using 3D-printed temporal bone models.After training,both groups completed a 2 h cadaveric temporal bone dissection to perform mastoidectomy.The primary outcome measure was the task-based checklist(TBC)score,which was comparatively analyzed during three phases:VR simulation training,3D-printed model dissection,and cadaveric dissection.Results In the continuous training,the TBC scores of both groups were higher than the previous one,and the difference was statistically significant(P<0.001),and the score difference between the two groups was also statistically significant(P<0.001).In the second training and the final temporal bone anatomy session,the TBC scores of the VR combined with 3D printing group were higher than those of the VR group,and the difference was statistically significant(P<0.001).Conclusion Both VR surgical simulation training and 3D printing technology demonstrated efficacy in enhancing otologic surgical skills,with superior performance observed in combined training.These findings suggest a synergistic reinforcement effect between the two modalities,providing evidence-based support for optimizing otologic surgical training paradigms.
