摘要
目的建立静脉用药调配中心(PIVAS)的数智化管理模式,以减少PIVAS配置不规范率。方法基于静脉用药调配中心信息系统,依托计划(Plan)、实施(Do)、检查(Check)、处理(Act)循环、真因分析等质量管理方法对静脉配置不规范率进行质控与改进。结合现状调查并依据80/20法则,判断“排药差错”和“配置剂量不准”是造成不规范事件的主要原因。其次,对“排药差错”和“配置剂量不准”进行对策制定。最终,构建数智化创新管理模式并投入实际运用。结果数智化管理模式实施后,围仓内配置阶段不规范事件发生率从3.001‰降至0.648‰(P<0.001);“排药差错”及“配置剂量不准”事件发生率分别从1.572‰降至0.183‰,1.080‰降至0.133‰(P<0.001)。此外,首批输液排药平均时长由28.7 min降至28.0 min(P<0.001)。结论静脉用药调配中心依托数智化管理的创新模式,优化配置流程,降低PIVAS围仓内配置不规范率并显著提升工作效率,以推动临床静脉输液的规范化、安全化、合理化。
OBJECTIVE Construct the intelligent management mode of centralized intravenous drug dispensing center to reduce the irregular allocation rate in the warehouse.METHODS Based on PIVAS information system,PDCA cycle,root cause analysis and other quality management methods,quality control and improvement were carried out.According to the 80/20 rule,“drug dispensing error”and“incorrect dose allocation”are the main causes of irregular events.Secondly,the countermeasures of“drug dispensing error”and“incorrect dose allocation”were formulated.Finally,the construction of digital intelligent innovation management model and put into practical application.RESULTS After the implementation of the digital intelligence management mode,the incidence of non-standard events in the configuration phase of the war ehouse decreased from 3.001‰to 0.648‰(P<0.001);the incidence of“drug dispensing error”and“incorrect dose allocation”decreased from 1.572‰to 0.183‰and 1.080‰to 0.133‰respectively(P<0.001).In addition,the average discharge time of the first batch of infusion drugs decreased from 28.7 min to 28.0 min(P<0.001).CONCLUSION The intravenous drug dispensing center relies on the in novative mode of digital intelligent management to optimize the allocation process,reduce the irregular allocation rate in the PIVAS warehouse and significantly improve the work efficiency,so as to promote the standardization,safety and rationalization of clinical intravenous infusion.
