摘要
总结了1例达芬奇机器人辅助腹腔镜下肾移植术后患者移植肾功能延迟恢复(DGF)的护理经验。护理要点包括:注重术前准备和心理护理;术后护理融入ERAS理念,优化生命体征监测、饮食管理、早期活动等措施;针对DGF,密切监测移植肾功能,血浆置换及血液滤过期间细致观察并处理并发症;防治尿路感染、下肢深静脉血栓等并发症的发生;采取保护性隔离等有效手段治疗甲流;重视患者的内心体验,加强心理护理及社会支持,鼓励患者自我成长,分享心理历程和成功经验。经积极救治和精心护理,术后48 d患者康复出院,术后随访4个月,移植肾功能良好。
Nursing experience of delayed recovery of graft kidney function in 1 patient after da Vinci robot-assisted laparoscopic kidney transplantation is summarized.Nursing points include:focusing on preoperative preparation and psychological care;integrating ERAS concepts into postoperative care;optimizing vital signs monitoring,dietary management,early activity and other measures;closely monitoring the transplanted renal function in response to DGF,carefully observing and dealing with complications during plasma exchange and haemofiltration;preventing and controlling complications such as urinary tract infections and lower limb deep vein thrombosis;and adopting protective isolation and other effective treatments for metritis;attaching importance to patients’inner experience,strengthening psychological care and social support,encouraging patients’self-growth,and sharing their psychological journey and successful experience.After active treatment and careful care,the patient recovered and was discharged from the hospital 48 days after the operation,and the transplanted kidney function was good in 4 months of postoperative follow-up.
