ApoB/ApoA-1比值、hs-CRP和CK-MB联合检测老年患者冠脉病变程度的诊断价值分析 被引量:1

摘要 目的:评价载脂蛋白B/载脂蛋白A1(Apo B/Apo A-1)比值、超敏C-反应蛋白(hs-CRP)和肌酸激酶同工酶(CK-MB)联合检测老年冠脉病变患者冠脉病变程度的诊断价值。方法:选取2021年1月—2023年1月商丘市第一人民医院收治的100例老年冠脉病变患者作为研究对象,检测其血清Apo B/Apo A-1比值、hs-CRP、CK-MB水平。以冠脉造影检查结果为标准,根据患者冠脉病变程度不同分组,比较其血清Apo B/Apo A-1比值、hs-CRP、CK-MB差异;分析Apo B/Apo A-1比值、hs-CRP、CK-MB水平与老年冠脉病变患者冠脉病变程度的相关性;绘制受试者工作者特征曲线(ROC)评估Apo B/Apo A-1比值、hs-CRP、CK-MB老年冠脉病变患者冠脉病变程度的诊断价值。结果:根据冠脉造影结果将本组患者分为轻度组31例、中度组37例和重度组32例;3组患者Apo B/Apo A-1比值、hs-CRP和CK-MB水平持续升高,组间比较差异均有统计学意义(F=28.392、8.842、13.868,P<0.001);Spearman相关性分析结果显示,Apo B/Apo A-1比值、hs-CRP、CK-MB水平与患者冠脉病变程度均呈正相关(r=0.618、0.384、0.471,P<0.001);ROC曲线提示,Apo B/Apo A-1比值、hs-CRP、CK-MB检测老年患者冠脉病变程度的AUC依次为0.815、0.714、0.745。结论:Apo B/Apo A-1比值、hs-CRP、CK-MB与老年患者冠脉病变程度呈正相关,可用于冠脉病变程度的临床诊断。 Objective:To evaluate the diagnostic value of apolipoprotein B/apolipoprotein A1(ApoB/ApoA-1)ratio,hypersensitive C-reactive protein(hs-CRP)and creatine kinase isoenzyme(CK-MB)in detecting the degree of coronary artery in elderly patients with coronary artery disease.Methods:100 elderly patients with coronary artery disease in the hospital from January 2021 to January 2023 were selected as the research object,and their serum ApoB/ApoA-1 ratio,hs-CRP and CK-MB levels were measured.Patients were divided into different groups according to the severity of coronary artery lesions,and the differences of serum ApoB/ApoA-1 ratio,hs-CRP and CK-MB were compared.Spearman analysis of the correlation between ApoB/ApoA-1 ratio,hs-CRP,CK-MB levels and the degree of coronary artery disease in elderly patients with coronary artery disease was conducted.The subject worker characteristic curve(ROC)was drawn to evaluate the diagnostic value of ApoB/ApoA-1 ratio,hs-CRP and CK-MB in detecting the degree of coronary artery disease in elderly patients with coronary artery disease.Results:According to the results of coronary angiography,the patients with coronary artery disease in this group were divided into three groups:31 patients in the mild group,37 patients in the moderate group and 32 patients in the severe group.From the mild group,the moderate group to the severe group,the level of ApoB/ApoA-1 ratio hs-CRP and CK-MB continued to increase,and the difference between the two groups was statistically significant(F=28.392,8.842,13.868;P<0.001).Spearman correlation analysis showed that the ratio of ApoB/ApoA-1,the levels of hs-CRP and CK-MB were positively correlated with the degree of coronary artery disease(r=0.618,0.384,0.471;P<0.001).The AUC of the ratio of ApoB/ApoA-1,hs-CRP and CK-MB in detecting the degree of coronary artery disease in elderly patients were 0.815,0.714 and 0.745 respectively.Conclusion:ApoB/ApoA-1 ratio,hs-CRP,CK-MB have positive correlation with the degree of coronary artery disease in elderly patients,which can be used for clinical diagnosis of coronary artery disease.
