摘要
目的:探讨胸部定量CT在慢性阻塞性肺疾病(COPD)患者肺功能评估中的价值。方法:选取2022年5月—2024年5月寿光市马店医院收治的80例COPD患者为研究对象,均开展胸部定量CT检查及肺功能检查,并依据慢性阻塞性肺疾病全球倡议(GOLD)分期将COPD患者分为轻度与重度两组,比较两组深吸气末肺容积(Vin)、呼气末肺容积(Vex)、肺气肿指数(EI)、CT空气潴留指数(ATI)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC水平差异;并采用Pearson相关性分析胸部定量CT参数与肺功能指标间的相关性。结果:重度COPD患者Vin、Vex、EI、ATI高于轻度COPD患者(P<0.05);重度COPD患者FVC、FEV1、FEV1/FVC低于轻度COPD患者(P<0.05);经Pearson相关性分析显示,Vin、Vex、EI、ATI与FVC、FEV1、FEV1/FVC呈负相关(P<0.05)。结论:胸部定量CT在COPD患者肺功能评价中具有较高价值,与传统肺功能检查存在高度相关性,利于更好明确肺功能状态,为疾病的诊疗提供参考。
Objective To explore the value of chest quantitative CT in assessing lung function in patients with chronic obstructive pulmonary disease(COPD).Methods Eighty patients with COPD admitted to Shouguang City Madian Hospital from May 2022 to May 2024 were selected as the study objects.Quantitative chest CT examination and lung function examination were conducted for all patients,and COPD patients were divided into mild and severe groups according to the Global Initiative for Chronic Obstructive Lung Disease(GOLD)staging.The levels of full inspiratory volume(Vin),full expiratory volume(Vex),emphysema index(EI),CT air-trapping index(ATI),forced lung capacity(FVC),forced expiratory volume in 1 second(FEV1)and FEV1/FVC were compared between the two groups.Pearson correlation was used to analyze the correlation between quantitative chest CT parameters and lung function indexes.Results Vin,Vex,EI and ATI in patients with severe COPD were higher than those with mild COPD(P<0.05).FVC,FEV1 and FEV1/FVC in patients with severe COPD were lower than those with mild COPD(P<0.05).According to Pearson correlation analysis,Vin,Vex,EI and ATI were negatively correlated with FVC,FEV1 and FEV1/FVC(P<0.05).Conclusions Chest quantitative CT has high value in evaluating lung function in COPD patients,and is highly correlated with traditional lung function tests,which is benefificial for better clarifying lung function status and providing reference for disease diagnosis and treatment.
