摘要
目的:观察参枣养心汤双心同治法治疗慢性心力衰竭(CHF)合并焦虑障碍的临床效果。方法:选取茂名市中医院心血管病科2021年1月—2023年12月收治的60例CHF合并焦虑障碍患者,随机分为对照组、治疗组,各30例。对照组给予一般治疗及心力衰竭规范化治疗,治疗组在对照组基础上加服参枣养心汤。比较两组患者中医证候积分、汉密尔顿焦虑量表评分、N末端脑钠肽前体水平、6分钟步行试验距离、心率变异性参数。结果:治疗1个月后,两组中医证候积分、汉密尔顿焦虑量表评分降低,治疗组比对照组低(P<0.05);治疗1个月后,两组N末端脑钠肽前体水平下降,6 min步行距离延长,每5 min节段窦性R-R间期的平均值标准差、正常窦性心搏间期的标准差数值升高,治疗组优于对照组(P<0.05)。结论:参枣养心汤双心同治法治疗CHF合并焦虑障碍的效果确切,能改善患者心力衰竭症状,增强运动耐力,改善焦虑状态,提高心率变异性参数。
Objective:To observe the clinical effect of Shenzao Yangxin decoction Shuangxin Tongzhi on chronic heart failure(CHF)complicated with anxiety disorder.Methods:Sixty patients with CHF complicated with anxiety disorder admitted to the department of cardiovascular of Maoming Hospital of Traditional Chinese Medicine from January 2021 to December 2023 were randomly divided into control group and treatment group,with thirty patients in each group.The control group was given general treatment and standardized treatment of heart failure,and the treatment group was given Shenzao Yangxin decoction on the basis of the control group.Traditional Chinese medicine syndrome score,Hamilton anxiety scale score,N-terminal brain natriuretic peptide precursor level,6-minute walking test distance and heart rate variability parameters were compared between the two groups.Results:After 1 month of treatment,traditional Chinese medicine syndrome score and Hamilton anxiety scale score of both groups were decreased,and the treatment group was lower than the control group(P<0.05).After 1 month of treatment,the level of N-terminal brain natriuretic peptide precursor decreased,the 6-min walking distance extended,and the mean standard deviation of sinus R-R interval and the standard deviation of normal sinus beat interval increased every 5 min in both groups,and the treatment group was better than the control group(P<0.05).Conclusion:Shenzao Yangxin decoction Shuangxin Tongzhi has the exact effect of treating CHF with anxiety disorder,which can improve the symptoms of heart failure,enhance exercise endurance,improve anxiety state and increase heart rate variability parameters.
