摘要
目的探究中频脉冲电刺激联合新斯的明对分娩镇痛产妇持续性产后尿潴留(PUR)的治疗效果。方法选取信丰县妇幼保健院2022年1月-2023年12月发生持续性PUR的70例分娩镇痛产妇,经随机数字表法分为对照组(35例)与观察组(35例),对照组给予新斯的明治疗,观察组在其基础上联合中频脉冲电刺激治疗,比较两组临床疗效、治疗后排尿情况(首次排尿时间、首次排尿后残余尿量)、尿潴留严重程度、性功能情况[女性性功能调查问卷(FSFI)]。结果观察组治疗总有效率高于对照组(P<0.05);与对照组比较,观察组治疗后首次排尿时间更短,首次排尿后残余尿量更少(P<0.05)。治疗后,两组尿潴留严重程度比较,差异有统计学意义(P<0.05),其中观察组轻度与无尿潴留患者占比多于对照组(P<0.05);观察组治疗后FSFI评分(性欲、性唤起、阴道湿润、性高潮、性满意度、性交痛)高于对照组(P<0.05)。结论中频脉冲电刺激联合新斯的明在分娩镇痛产妇持续性PUR治疗中具有确切疗效,可缩短患者的自行排尿时间,减少其残余尿量,改善患者尿潴留严重程度,提高其性生活质量。
Objective To explore the therapeutic effect of medium frequency electrical stimulation combined with neostigmine on persistent postpartum urinary retention(PUR)in parturients undergoing labor analgesia.Methods A total of 70 parturients with persistent PUR who underwent labor analgesia in Xinfeng County Maternal and Child Health Hospital from January 2022 to December 2023 were selected and divided into control group(35 parturients)and observation group(35 parturients)by random number table method.The control group was treated with neostigmine,and the observation group was treated with medium frequency electrical stimulation on the basis of neostigmine.The clinical efficacy,urination after treatment(first urination time,residual urine volume after first urination),severity of urinary retention,sexual function[female sexual function index(FSFI)]were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).Compared with the control group,the first urination time after treatment in the observation group was shorter,and the residual urine volume after the first urination was less(P<0.05).After treatment,there was significant difference in the severity of urinary retention between the two groups(P<0.05),and the proportion of patients with mild and no urinary retention in the observation group was higher than that in the control group(P<0.05).After treatment,the FSFI score(libido,sexual arousal,vaginal moist,orgasm,sexual satisfaction,dyspareunia)in the observation group was higher than that in the control group(P<0.05).Conclusion Medium frequency electrical stimulation combined with neostigmine has a definite effect in the treatment of persistent PUR in parturients with labor analgesia,which can shorten the patient's self-urination time,reduce the residual urine volume,improve the severity of urinary retention and improve the quality of sexual life.
