摘要
目的探究中药熏洗联合改良内括约肌侧切术治疗陈旧性肛裂的效果。方法选取2021年7月—2022年12月在单县中心医院接受手术治疗的陈旧性肛裂患者60例,根据随机数字表法分为联合组与对照组,各30例。对照组采用传统手术治疗,联合组采用改良内括约肌侧切手术配合中药熏洗治疗;观察两组治疗效果。结果联合组治疗总有效率93.33%(28/30)高于对照组的73.33%(22/30)(P<0.05);联合组术后创面愈合时间、水肿消失时间、疼痛缓解时间均短于对照组(P<0.05);两组患者肛管最大收缩压(MSP)水平差异无统计学意义(P>0.05),联合组肛管静息压(ARP)、直肠静息压(RRP)水平均低于对照组(P<0.05);联合组不良反应发生率20.00%(6/30)低于对照组的53.33%(16/30)(P<0.05);联合组生活质量评分优于对照组(P<0.05)。结论改良内括约肌侧切手术联合中药熏洗治疗陈旧性肛裂可以在极大程度上减轻患者的痛苦,提升效果,加快患者恢复速度,值得在临床上广泛应用。
Objective To investigate the effect of Chinese medicine fumigation combined with improved internal sphincter lateral resection in the treatment of chronic anal fissure.Methods A total of 60 patients with chronic anal fissure who received surgical treatment in Shanxian Central Hospital from July 2021 to December 2022 were selected and divided into combined group and control group according to random number table method,with 30 cases in each group.The control group was treated with traditional surgery,and the combined group was treated with Chinese medicine fumigation combined with improved internal sphincter lateral resection.The therapeutic effect of the two groups was observed.Results The total effective rate of combined group was 93.33%(28/30),which was higher than that of control group(73.33%,22/30)(P<0.05).Postoperative wound healing time,edema disappearance time and pain relief time in combined group were shorter than those in control group(P<0.05).There was no significant difference in the maximum anal canal systolic blood pressure(MSP)between the two groups(P>0.05),and the anal canal resting pressure(ARP)and rectal resting pressure(RRP)in the combined group were lower than those in the control group(P<0.05).The incidence of adverse reactions in combined group was 20.00%(6/30),which was lower than that in control group(53.33%,16/30)(P<0.05).The quality of life score of the combined group was better than that of the control group(P<0.05).Conclusion For patients with chronic anal fissure,Chinese medicine fumigation combined with improved internal sphincter lateral resection can significantly reduce the pain of patients,improve the therapeutic effect and accelerate the recovery rate,and is worthy of wide application in clinical practice.
