摘要
目的:比较腹腔镜下根治术与传统开腹术治疗结直肠癌的临床疗效。方法:选取2022年1月—2023年1月在南阳市中心医院进行手术治疗的结直肠癌患者110例,根据随机数表法分为对照组(实施传统开腹术)及观察组(实施腹腔镜根治术),每组各55例。观察两组的手术相关指标、胃肠功能、炎症因子、并发症发生情况。结果:与对照组相比,观察组的手术时间更长(P<0.05),术中出血量更少(P<0.05),住院时间更短(P<0.05);观察组的排气、肠鸣音恢复、排便及进食时间均短于对照组(P<0.05);术后,观察组的C反应蛋白(CRP)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)水平低于对照组(P<0.05);术后,与对照组相比,观察组的丙二醛(MDA)及皮质醇(Cor)水平更低(P<0.05),超氧化物歧化酶(SOD)水平更高(P<0.05);观察组的并发症发生率低于对照组(P<0.05)。结论:与传统开腹术相比,腹腔镜下根治术治疗结直肠癌可更有效地减轻炎症反应及应激反应,有利于胃肠功能的恢复,降低并发症的发生率,值得临床推广。
Objective:To compare the clinical efficacy of laparoscopic radical resection and traditional open surgery for colorectal cancer.Methods:A total of 110 colorectal cancer patients who underwent surgery at Nanyang Central Hospital from January 2022 to January 2023 were selected and randomly divided into a control group(traditional open surgery,n=55)and an observation group(laparoscopic radical resection,n=55)using a random number table.Surgical-related indicators,gastrointestinal function,inflammatory factors,and complications were analyzed.Results:Compared with the control group,the observation group had a longer operation time,less intraoperative blood loss,and a shorter hospital stay(P<0.05).The time to first flatus,bowel sounds recovery,defecation,and oral intake in the observation group were significantly shorter than those in the control group(P<0.05).Postoperatively,the observation group exhibited lower levels of CRP,IL-6,and TNF-α(P<0.05),as well as reduced MDA and Cor levels and higher SOD levels(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:Compared with traditional open surgery,laparoscopic radical resection for colorectal cancer more effectively alleviates inflammatory and oxidative stress responses,promotes gastrointestinal function recovery,and reduces complications,making it worthy of clinical promotion.
