摘要
本文在回顾历史和分析中医肿瘤学科发展现状的基础上,提出以往这种以丢掉中医辨病论治特色为代价的辨证论治,长期对经典著作的轻视,就是造成中医肿瘤学科发展瓶颈的深层原因。指出《黄帝内经》(简称《内经》)提示风邪、寒邪入里成瘤的病因学说值得深入研究,基于风为百病之长的中医共识,认为风邪是肿瘤发生发展的重要因素,心神不安是邪风入里的内在条件,多种因素综合是邪风入里成瘤的病机特点,瘙痒是风邪入里成瘤的症状依据。风邪入里成瘤实质是风邪与寒、热、湿、痰、气机、津液、瘀血等夹杂致病,人体阴阳的平衡就是被风邪的变化多端、肆无忌惮的特点所破坏的。表现在症候上是主次先后变化快,涉及脏腑经络多,病情轻重差异大,提出了“风邪入里成瘤说”;《内经》强调肿瘤的形成过程——寒邪凝滞,入里成积;《内经》肿瘤病机强调寒热相搏、津液涩渗;《金匮要略》的肺痿、肠痈,与肺癌、大肠癌关系密切,具有可比性,是恶性肿瘤辨病论治的典范;肿瘤可从六经论治;经方治疗恶性肿瘤大有可为;《神农本草经》在治疗恶性肿瘤方面潜力无限;经方在恶性肿瘤的防治养结合方面越来越受到重视。从而得出激活经典是突破中医肿瘤学科发展瓶颈必由之路的结论。
Based on a comprehensive review of the historical evolution and current status of traditional Chinese medicine(TCM)oncology,this paper identifies two fundamental factors contributing to the developmental bottleneck in this discipline:the overemphasis on syndrome differentiation at the expense of TCM disease differentiation characteristics,and the persistent neglect of classical TCM literature in contemporary practice and research.Huangdi Neijing proposes the theory of pathogenic wind and cold invading the body,which is worthy of in-depth investigation.Based on the TCM consensus that“wind is the leading cause of all diseases”,this study suggests that pathogenic wind plays a significant role in the initiation and progression of tumors.Furthermore,the disturbance of the heart spirit is identified as the intrinsic condition that facilitates the invasion of pathogenic factors into the body.The pathogenic mechanism underlying tumor formation involves a combination of multiple factors,characterized by the internalization of pathogenic factors,with pruritus serving as a symptomatic indicator of this process.The pathogenic mechanism of pathogenic wind-induced tumor formation is characterized by the complex interaction of pathogenic wind with other factors,including cold,heat,dampness,phlegm,qi dysregulation,body fluid imbalance,and static blood.The dynamic and unpredictable nature of pathogenic wind disrupts the equilibrium of yin and yang within the human body,thereby creating a pathological environment conducive to tumor development.In terms of clinical manifestations,the symptoms exhibit rapid alternation between primary and secondary manifestations,frequently involving multiple zang-fu organs,meridians,and collaterals,with significant variability in disease severity.This study proposes the hypothesis that“pathogenic wind invading the body contributes to tumor formation”.Huangdi Neijing elucidates the process of tumorigenesis,emphasizing the congealing and accumulation of pathogenic cold in the interior,as well as the interplay between cold and heat and the obstruction of body fluids as key mechanisms in tumor pathogenesis.Furthermore,Synopsis of Prescriptions of the Golden Chamber establishes a correlation between lung atrophy and intestinal abscess with modern concepts of lung cancer and colorectal cancer,respectively,providing a valuable framework for disease identification and treatment of malignant tumors.The therapeutic approach to tumors can be guided by the theory of six-meridian differentiation,and classical formulas demonstrate significant potential in the management of malignant tumors.Additionally,Shen Nong's Herbal Classic offers substantial insights into tumor treatment.In recent years,classical formulas have garnered increasing attention for their role in the prevention and treatment of malignant tumors,highlighting their enduring relevance in contemporary oncology practice.Thus,it can be concluded that revitalizing classical theories is an essential pathway to overcome developmental bottlenecks in the discipline of TCM oncology.
