摘要
目的探讨超声引导下骶管阻滞联合独活寄生汤在寒湿阻络型腰腿痛中的应用效果。方法选取2023年7月至2024年7月萍乡市妇幼保健院针灸康复科收治的60例寒湿阻络型腰腿痛患者作为研究对象,按照随机数字表法分为对照组和研究组,每组30例。对照组给予口服独活寄生汤治疗,研究组在口服独活寄生汤基础上联合超声引导下骶管阻滞治疗。比较两组患者的治疗时间、腰腿痛症状、Oswestry功能障碍指数(ODI)及视觉模拟评分法(VAS)评分。结果治疗第7、14天,研究组的VAS、ODI评分低于对照组,差异有统计学意义(P<0.05)。研究组的治疗时间短于对照组,差异有统计学意义(P<0.05)。结论超声引导下骶管阻滞联合独活寄生汤治疗寒湿阻络型腰腿痛的临床效果优于单纯口服独活寄生汤,可有效缓解疼痛,改善功能障碍,缩短治疗时间。
Objective To investigate the application effect of ultrasound-guided caudal epidural block combined with Duhuo Jisheng Decoction in cold-damp obstruction type lumbocrural pain.Methods A total of 60 patients with cold-damp obstruction type lumbocrural pain in the Department of Acupuncture and Moxibustion Rehabilitation of Pingxiang Maternity and Child Care from July 2023 to July 2024 were selected as research objects,and they were divided into control group and study group according to random number table method,with 30 cases in each group.The control group was treated with oral Duhuo Jisheng Decoction,and the study group was treated with ultrasound-guided caudal epidural block on the basis of oral Duhuo Jisheng Decoction.The duration of treatment,symptoms of back and leg pain,Oswestry disability index(ODI)and visual analogue scale(VAS)were compared between the two groups.Results On the 7th and 14th day of treatment,VAS and ODI scores of the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).The duration of treatment of the study group was shorter than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The clinical effect of ultrasound-guided caudal epidural block combined with Duhuo Jisheng Decoction in the treatment of cold-damp obstruction type lumbocrural pain is better than that of oral Duhuo Jisheng Decoction alone,which can effectively relieve pain,improve dysfunction and shorten the duration of treatment.
