摘要
目的检索、评价并整合老年2型糖尿病(type 2 diabetes mellitus,T2DM)患者过渡期用药安全管理的循证证据,为患者用药安全管理提供依据。方法遵循“6S”模型检索计算机决策支持系统、指南网站及各数据库中与糖尿病患者用药管理相关的证据,检索时限为建库至2023年11月。由2名研究者根据AGREEⅡ、CASE及JBI循证中心的评价标准和证据分级系统评价文献质量,汇总证据内容。结果共纳入文献13篇,包括8篇指南、3篇专家共识和2篇推荐实践。归纳出评估、治疗方案、安全用药、院内用药安全管理措施和医院-家庭过渡期管理5个方面共40条证据。结论该研究为医务人员提供了老年T2DM患者用药安全管理的循证依据。医务人员应加强多学科团队合作,应用证据时审慎考虑患者不同阶段的特征与个体差异,确保用药的有效性和安全性。
Objective To evaluate and integrate evidence on medication safety management for older patients with T2DM throughout transitional periods.Methods We utilized the'6S'model to search for relevant literature from computerized decision support systems,guideline websites,and databases up to November 2023.Two researchers assessed the literature using AGREE II,CASE,and JBI Evidence Centre criteria and summarized the findings.Results Thirteen publications were included:8 guidelines,3 expert consensuses,and 2 recommended practices.The evidence was categorized into five areas(40 entries):assessment,treatment options,safe medication use,in-hospital management,and hospital-to-home transitions.Conclusion This study provides healthcare professionals with an evidence-based framework for managing medications in elderly T2DM patients.It highlights the importance of multidisciplinary teamwork and the consideration of patient characteristics to ensure effective and safe medication management.
