摘要
目的 研究新型单侧双通道内镜技术(UBE)结合改良微创内固定术治疗胸腰段脊柱结核(STB)的临床效果。方法 选取2024年1月至10月九江市第三人民医院确诊STB患者84例,随机数字表分为对照组和观察组各42例。对照组行传统后路手术,观察组采用UBE技术+改良置钉法。比较两组围术期指标、术后脊柱功能、疼痛、血沉(ESR)、Oswestry功能障碍指数(ODI)及并发症。结果 治疗后,观察组在出血量、输血量、手术时间、住院时间上均优于对照组(P<0.05);治疗后,观察组腰椎日本骨科学会(JOA)评分更高,视觉模拟评分法(VAS)评分、ESR、ODI更低(P<0.05),观察组治疗后并发症总发生率9.25%低于对照组14.29%,差异无统计学意义(P>0.05)。结论 新型UBE技术结合微创内固定术减少手术创伤,加速康复,缓解疼痛,促进脊柱功能恢复,安全性良好。
Objective To study the therapeutic outcome of unilateral biportal endoscopy(UBE)combined with modified minimally invasive internal fixation in the treatment of thoracolumbar spinal tuberculosis(STB).Methods From January to October 2024,Eighty-four STB patients diagnosed at the Third People's Hospital of Jiujiang City were recruited and randomly divided into the control group and observation group,with 42 cases in each group.Patients in the control group were treated with a traditional surgery via the posterior approach,while those in the observation group were treated with UBE combined with modified screw placement method.The perioperative indicators,postoperative spinal function,pain,erythrocyte sedimentation rate(ESR),Oswestry disability index(ODI),and complications were compared between the two groups.Results Blood loss,transfusion volume,operation time and hospital stay were significantly lower in the observation group than those of the control group(P<0.05).After treatment,the Japanese Orthopaedic Association(JOA)score of lumbar spine in the observation group was significantly higher,and the visual analogue scale(VAS),ESR and ODI were significantly lower than those of the control group(P<0.05).The total incidence of complications in the observation group after treatment was similar with that of the control group(9.25%VS 14.29%,P>0.05).Conclusion The novel UBE technology combined with minimally invasive internal fixation reduces surgical trauma,accelerates recovery,alleviates pain,promotes the restoration of spinal function,and has a good safety.
