摘要
目的:观察筋骨消痛丸联合膝关节脉冲射频治疗肝肾亏虚型膝骨关节炎的临床疗效。方法:选择2021年5月至2022年5月河南省中医院收治的100例肝肾亏虚型膝骨关节炎患者,依据随机数字表法分为对照组和治疗组,每组各50例。治疗组给予筋骨消痛丸外敷联合膝关节脉冲射频治疗,对照组给予膝关节脉冲射频治疗。观察两组患者的临床疗效,检测两组患者治疗前后肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素(interleukin,IL)-1、IL-7R、IL-1β、核转录因子-κB受体活化因子配体(nuclear transcription factor-κB receptor activating factor ligand,RANKL)、软骨寡聚基质蛋白(cartilage oligomer matrix protein,COMP)、骨保护素(osteopagin,OPG)、Ⅱ型胶原羧基端端肽(typeⅡcollagen carboxyl end-terminal peptide,CTX-Ⅱ)水平,比较两组患者治疗前后膝关节屈-伸活动角度、疼痛数字评价量表(numerical rating scale,NRS)评分、膝关节运动功能Lysholm量表(knee motor function Lysholm scale,LKSS)、西安大略麦克马斯特大学骨关节炎指数(Western ontario and mcmaster university osteoarthritis index,WOMAC)及临床症状评分变化情况。结果:治疗组有效率为96.0%,高于对照组的80.0%(P<0.05)。两组患者治疗后IL-1、TNF-α、IL-1β水平低于本组治疗前,IL-7R水平高于本组治疗前,治疗后组间比较,差异具有统计学意义(P<0.05)。两组患者治疗后RANKL、COMP、CTX-Ⅱ低于本组治疗前(P<0.05),OPG高于本组治疗前,治疗后组间比较,差异具有统计学意义(P<0.05)。两组患者治疗后NRS评分低于本组治疗前,关节活动度高于本组治疗前,治疗后组间比较,差异具有统计学意义(P<0.05)。两组患者治疗后WOMAC、临床症状评分低于本组治疗前,LKSS评分高于本组治疗前,治疗后组间比较,差异具有统计学意义(P<0.05)。结论:筋骨消痛丸联合膝关节脉冲射频治疗肝肾亏虚型膝骨关节炎,可抑制炎症因子水平,调节机体骨代谢,减少患者软骨及滑膜损伤,缓解疼痛程度,提高关节活动度及关节功能。
Objective:To observe the clinical efficacy of Jingu Xiaotong Pills combined with knee joint pulsed radiofrequency on knee osteoarthritis with syndrome of deficiency of liver and kidney.Methods:A total of 100 patients with liver-kidney deficiency type knee osteoarthritis admitted to Henan Provincial Hospital of TCM from May 2021 to May 2022 were selected and randomly divided into the control group and the treatment group,with 50 cases in each group.The treatment group was given Jinggu Xiaotong Pills combined with knee joint pulsed radiofrequency treatment,while the control group was given knee joint pulsed radiofrequency treatment alone.The clinical efficacy of the two groups was observed.The levels of tumor necrosis factor-α(TNF-α),interleukin-1(IL-1),interleukin-7 receptor(IL-7R),interleukin-1β(IL-1β),nuclear transcription factor-KB receptor activating factor ligand(RANKL),cartilage oligomer matrix protein(COMP),osteoprotegerin(OPG),and type II collagen carboxyl end-terminal peptide(CTX-II)were detected before and after treatment in both groups.The changes in knee flexion-extension range of motion,numerical rating scale(NRS)score for pain,knee motor function Lysholm scale(LKSS),Western Ontario and McMaster University Osteoarthritis Index(WOMAC),and clinical symptom scores before and after treatment were compared between the two groups.Results:The effective rate of the treatment group was 96.0%,which was higher than that of the control group(80.0%)(P<0.05).After treatment,the levels of IL-1,TNF-α,and IL-1βin both groups were lower than those before treatment,while the level of IL-7R was higher than that before treatment,and there were statistically significant differences between the two groups(P<0.05);the levels of RANKL,COMP,and CTX-Ⅱin both groups were lower than those before treatment(P<0.05),while the level of OPG was higher than that before treatment,and there were statistically significant differences between the two groups(P<0.05);the NRS scores of both groups were lower than those before treatment,while the joint range of motion was higher than that before treatment,and there were statistically significant differences between the two groups(P<0.05);the WOMAC and clinical symptom scores of both groups were lower than those before treatment,while the LKSS scores were higher than those before treatment,and there were statistically significant differences between the two groups(P<0.05).Conclusion:For patients with knee osteoarthritis of liver-kidney deficiency type,Jingu Xiaotong Pills combined with knee joint pulsed radiofrequency can inhibit inflammation,regulate bone metabolism,reduce cartilage and synovial membrane damage,relieve pain,improve joint range of motion and joint function,and has a definite therapeutic effect.
