摘要
白癜风(Vitiligo)是一种常见的原发性色素脱失性疾病,主要影响皮肤和黏膜,表现为局部或广泛的色素完全缺失。它也被称为“白癜”,其临床特征包括皮肤颜色减退、变白,边界清晰,且患者通常没有自觉症状。目前,该疾病的发病机制仍然不明确。西医认为白癜风的发病与遗传,自身免疫,黑色素细胞自毁,氧化应激等相关;中医学认为白癜风的发生与脏腑功能失调、外感六淫密切相关。目前西医多以口服或外用药物、光疗及手术等治疗该病,短期疗效显著,但易反复且有诸多不良反应;中医在治疗上多以补肝肾、祛风湿、疏肝气、化瘀血为总原则。该文系统梳理白癜风近年来的发病机制和治疗进展,以期为未来的研究工作提供有价值的参考和启示。
Vitiligo is a common primary pigmentary disorder,primarily affecting the skin and mucous membranes,characterized by local or widespread complete loss of pigment.It is also known as"leukoderma',with clinical features including reduced skin color,whitening and clear boundaries,and the patients typically have no subjective symptoms.The pathogenesis of this disease is still not clear.Western medicine believes that the onset of vitiligo is related to genetics,autoimmune,melanocyte self-destruction and oxidative stress,while traditional Chinese medicine considers the occurrence of vitiligo is closely related to the dysfunction of Zang-Fu organs and the invasion of exogenous pathogenic factors.Currently,Western medicine mainly treats this disease with oral or topical medications,phototherapy and surgery,with significant short-term effects but being prone to relapse and many adverse reactions.Traditional Chinese medicine treatment mostly follows the general principles of nourishing liver and kidney,dispelling dampness and wind,soothing liver Qi and resolving blood stasis.This article aimed to summarize the recent advances in the pathogenesis and treatment of vitiligo,in hope of providing valuable references and insights for future research work.
