摘要
目的研究旨在通过分析某三级医院住院死亡病例的特点,为提升医疗服务质量、优化医疗资源配置、制定针对性的疾病防治措施提供科学依据。方法从医院病案系统中调取某院2014年1月1日-2023年12月31日所有出院患者资料,采用Excel 2016和SPSS 27.0对死亡患者的年龄、性别和疾病种类等特征进行分析,死亡原因分类采用国际疾病分类ICD-10。结果医院总出院人数为1518572人,其中死亡患者5315人,病死率为0.35%,总体病死率随着年份的增长呈现下降的趋势。男性占比64.65%,病死率0.23%;女性占比35.35%,病死率0.12%,男性患者病死率要高于女性患者,差异具有统计学意义。60岁以上组死亡人数最多,占比达到0.49%。死亡疾病谱前3位的是循环系统疾病(33.64%)、呼吸系统疾病(18.23%)和肿瘤(16.35%)。脑栓塞、重症肺炎和肺恶性肿瘤分别是循环系统、呼吸系统和肿瘤死亡疾病的主要病种。结论60岁以上老年人是死亡患者的主要群体,脑栓塞、脑出血、重症肺炎、慢性阻塞性肺病、肺恶性肿瘤和卵巢恶性肿瘤是导致死亡的重点疾病。医院和当地卫生部门需加强老年人保健及相关学科建设,以及预防老年人意外伤害相关知识的宣传教育。
Objectives This study aims to provide a scientific basis for improving the quality of medical services,optimizing the allocation of medical resources,and formulating targeted disease prevention and control measures by analyzing the characteristics of inpatient deaths in a tertiary hospital.Methods All discharged patients'records from a certain hospital from January 1,2014 to December 31,2023 were retrieved from the hospital medical record system.Excel 2016 and SPSS 27.0 were used to analyze the age,gender,and disease type of the deceased patients,and the cause of death was classified using the International Classification of Diseases ICD-10.Results The total number of patients discharged from the hospital was 1,518,572,of which 5,315 died,with a mortality rate of 0.35%.The overall mortality rate showed a downward trend with the increase of years.Males accounted for 64.65% with a mortality rate of 0.23%;females accounted for 35.35% with a mortality rate of 0.12%.The mortality rate of male patients was higher than that of female patients,and the difference was statistically significant.The number of deaths was highest in the group aged 60 and above,accounting for 0.49%.The top three causes of death were circulatory system diseases(33.64%),respiratory system diseases(18.23%)and tumors(16.35%).Cerebral embolism,severe pneumonia and lung malignant tumors were the main types of death from circulatory system,respiratory system and tumor diseases respectively.Conclusions Elderly people over 60 years old are the main group of deaths,and cerebral embolism,cerebral hemorrhage,severe pneumonia,chronic obstructive pulmonary disease,lung malignant tumors and ovarian malignant tumors are the main causes of death.Hospitals and local health departments need to strengthen the construction of elderly care and related disciplines,as well as knowledge related to preventing accidental injuries to the elderly.
