摘要
目的:探讨钝针扣眼顺血流穿刺技术联合目标管理对尿毒症患者自体动静脉内瘘(AVF)并发症的影响.方法:选取 2023 年 1 月至 2023 年 6 月本院收治的 60 例尿毒症血液透析患者作为研究对象.根据随机数字表法,将其分为观察组(n=30,接受钝针扣眼顺血流穿刺技术联合目标管理干预)和对照组(n=30,接受锐针穿刺技术联合目标管理干预).比较两组患者穿刺相关情况、护理舒适度以及AVF相关并发症发生率.结果:观察组患者按压止血时间、穿刺渗血率、穿刺疼痛程度均低于对照组,一次性穿刺成功率高于对照组,差异均有统计学意义(P<0.05).观察组患者 Kolcaba 舒适状况量表(GCQ)各维度评分均高于对照组,差异均有统计学意义(P<0.05).观察组患者AVF相关并发症发生率低于对照组,差异均有统计学意义(P<0.05).结论:钝针扣眼顺血流穿刺技术联合目标管理能够改善尿毒症患者AVF穿刺情况、提高护理舒适度、减少并发症,提高治疗效果.
Objective:To explore the effect of blunt needle buttonhole technique combined with target management on complications of native arteriovenous fistula(AVF)in patients with uremia.Methods:Sixty patients with uremia who underwent hemodialysis at our hospital from January to June 2023 were selected as research subjects.According to random number table method,they were divided into observation group(n=30,received blunt needle buttonhole technique combined with target management intervention)and control group(n=30,received sharp needle puncture combined with target management intervention).Puncture-related situations,comfort level of nursing and the incidence of AVF-related complications were compared between the two groups.Results:The time for compression hemostasis,puncture bleeding rate,and puncture pain degree of observation group were less than those of control group,and the one-time puncture success rate of observation group was higher than that of control group,and the differences were statistically significant(P<0.05).The Kolcaba's general comfort questionnaire(GCQ)scores of observation group were higher than those of control group,and the differences were statistically significant(P<0.05).The incidence of AVF-related complications of observation group were lower than those of control group,and the dfferences were statistically significant(P<0.05).Conclusion:Blunt needle buttonhole technique combined with target management can improve AVF puncture,promote comfort level,and reduce the incidence of complications,enhance the effectiveness of the treatment.
