摘要
目的分析磁共振成像(MRI)影像学特征与腰椎间盘突出症(lumbar disc herniation LDH)患者Oswestry功能障碍指数(ODI)评分的相关性。方法选取2022年3月至2024年6月本院收治的93例LDH患者,参照ODI评分系统评估患者功能障碍程度,分为轻度组(ODI评分<15分)、中度组(ODI评分15-30分)、重度组(ODI评分>30分);患者均接受MRI检查,记录MRI影像学特征[主要包括Pfirrmann分级、Modic分型、矢状径指数(SI)、矢状径/横径指数(STI)、横径指数(TI)、神经根及突出物夹角(AN角)];比较三组患者一般资料、MRI影像学特征;经Kendall's tau-b相关系数检验MRI影像学特征与LDH患者ODI指数的相关性。结果93例LDH患者经评估,轻度、中度、重度功能障碍各有33例、36例、24例;ODI评分分别为(12.24±2.05)分、(22.65±3.16)分、(34.45±3.91)分。中度组、重度组Pfirrmann分级评分、SI、STI值高于轻度组,TI、AN角低于轻度组,且重度组Pfirrmann分级评分、SI、STI值高于中度组,TI、AN角低于中度组(均P<0.05)。三组Modic分型比较,无统计学差异(P>0.05)。经Kendall's tau-b相关性分析,LDH患者ODI指数与Pfirrmann分级评分、SI、STI呈正相关(r>0,P<0.05),与TI、AN角呈负相关(r<0,P<0.05)。结论LDH患者ODI指数与MRI影像学特征存在相关性。
Objective To analyze the correlation between magnetic resonance imaging(MRI)features and Oswestry Disability Index(ODI)scores in patients with lumbar disc herniation(LDH).Methods A total of 93 patients with LDH who were admitted to our hospital from March 2022 to June 2024 were selected.The degree of dysfunction was assessed using the ODI scoring system,and the patients were divided into three groups:mild(ODI<15),moderate(ODI 15-30),and severe(ODI>30).All patients underwent MRI,and the MRI imaging features were recorded,including Pfirrmann grading,Modic staging,sagittal diameter index(SI),sagittal/transverse diameter index(STI),transverse diameter index(TI),and angle of nerve root entrapment and protrusion(AN angle).The general information and MRI features of the three groups were compared,and the correlation between MRI imaging features and ODI scores in LDH patients was assessed using Kendall's tau-b correlation coefficient.Results A total of 93 patients with LDH were classified into mild,moderate,and severe dysfunction groups,with 33,36,and 24 patients in each group,respectively.The ODI scores were(12.24±2.05),(22.65±3.16),and(34.45±3.91)for the mild,moderate,and severe groups,respectively.The Pfirrmann grading score,SI,and STI were significantly higher in the moderate and severe groups compared to the mild group,while the TI and AN angle were significantly lower.Furthermore,the Pfirrmann grading score,SI,and STI values were higher in the severe group than in the moderate group,while the TI and AN angles were lower in the severe group compared to the moderate group(P<0.05).No significant difference was observed between the three Modic staging groups(P>0.05).Kendall's tau-b correlation analysis revealed that the ODI index was positively correlated with the Pfirrmann grading score,SI,and STI(r>0,P<0.05),and negatively correlated with the TI and AN angle(r<0,P<0.05).Conclusion A significant correlation was found between the ODI index and MRI imaging features in patients with LDH.
