老年患者全麻诱导期靶控输注环泊酚或丙泊酚的对照临床实验研究

摘要 目的:探讨老年患者全麻诱导期靶控输注环泊酚或丙泊酚的安全性和临床效果,为老年患者全麻诱导期靶控输注选择合适的药物提供依据。方法:选取2022年8月—2023年8月于高州市中医院进行全麻手术的144例老年患者作为研究对象,随机分为环泊酚诱导组和丙泊酚诱导组,每组各72例。全麻诱导期分别靶控输注环泊酚、丙泊酚,比较两组患者诱导时间、研究药物总量、血浆靶浓度,各时间点生命体征指标及不良反应发生情况。结果:环泊酚诱导组研究药物总量、平均血浆靶浓度低于丙泊酚诱导组,差异均有统计学意义(t=14.115、16.135,P<0.05);气管插管前(T1)、气管插管后即刻(T2)、气管插管后5 min(T3)时,两组患者心率(HR)、平均动脉压(MAP)、舒张压(SBP)、收缩压(DBP)、脑电双频谱指数(BIS)、心脏指数(CI)、每搏输出量(SV)、心输出量(CO)指标水平均呈现下降趋势,每搏量变异度(SVV)指标水平均呈现上升趋势,差异均有统计学意义(t=5.084、5.147、5.007;5.903、12.184、13.648;4.979、4.503、3.819;14.655、11.273、9.199;0.382、0.187、0.294;5.062、10.406、14.149;2.972、5.704、7.995;2.784、2.903、3.277;3.555、6.601、15.493,P<0.05);环泊酚诱导组不良反应发生率低于丙泊酚诱导组,差异有统计学意义(χ^(2)=49.000,P<0.001)。结论:老年患者全麻诱导期靶控输注环泊酚和丙泊酚均能达到临床麻醉效果,输注环泊酚可显著减少药物的使用剂量,减少对患者生命体征的影响,降低不良反应发生率,安全性高。 Objective:To investigate the safety and clinical effect of targeted infusion of cypofol or propofol in the induction period of general anesthesia in elderly patients,and to provide evidence for the selection of appropriate drugs for the induction period of general anesthesia in elderly patients.Methods:A total of 144 elderly patients undergoing general anesthesia surgery in Traditional Chinese Medical Hospital of Gaozhou from August 2022 to August 2023 were selected as the study objects,and randomly divided into cyclopofol induction group and propofol induction group,with 72 cases in each group.During the induction period of general anesthesia,targeted infusion of cyclopofol and propofol were respectively administered,and the induction time,total amount of studied drugs and plasma target concentration,vital signs and adverse reactions at each time point of the two groups were compared.Results:The total drug amount and average plasma target concentration in cyclopofol induction group were lower than those in propofol induction group,and the differences were statistically significant(t=14.115,16.135;P<0.05).At T1,T2,and T3,HR,MAP,SBP,DBP,BIS,CI,SV,and CO indexes in both groups showed a downward trend,while SVV indexes showed an upward trend,with statistically significant difference(t=5.084,5.147,5.007;5.903,12.184,13.648,4.979,4.503,3.819,14.655,11.273,9.199,0.382,0.187,0.294,5.062,10.406,14.149,2.972,5.704,7.995,2.784,2.903,3.277,3.555,6.601,15.493;P<0.05).The incidence of adverse reactions in cyclopofol induced group was lower than that in propofol induced group,and the difference was statistically significant(χ^(2)=49.000,P<0.001).Conclusion:Both cipofol and propofol can achieve clinical anesthetic effects in elderly patients during the induction period of general anesthesia,but cipofol infusion can significantly reduce the dose of drugs,reduce the impact on patients’vital signs,and reduce the incidence of adverse reactions,with high safety and worthy of promotion.
