摘要
目的 观察加味二陈汤联合西药治疗老年社区获得性肺炎痰湿阻肺证的临床疗效。方法 选取2022年1月-2024年3月乐平市中医医院接受治疗的痰湿阻肺型老年社区获得性肺炎患者64例为研究对象,按照随机数字表法分为对照组和研究组,各32例。对照组应用常规西药治疗,研究组在对照组基础上应用加味二陈汤治疗。比较两组临床疗效、症状消除时间(咳嗽消失时间、气促消失时间、炎症吸收时间、体温恢复正常时间、啰音消失时间)、炎性细胞[降钙素原(PCT)、中性粒细胞比值(NEUT%)、白细胞计数(WBC)、C-反应蛋白(CRP)]水平、中医证候(咳嗽、发热、咳痰、胸闷、乏力)积分。结果 与对照组治疗总有效率的75.00%比较,研究组治疗总有效率更高,达96.88%(P<0.05);与对照组比较,研究组咳嗽消失时间、气促消失时间、炎症吸收时间、体温恢复正常时间、啰音消失时间更短(P<0.05);与治疗前比较,两组治疗后WBC、NEUT%、CRP、PCT水平均降低,且与对照组比较,研究组治疗后WBC、NEUT%、CRP、PCT水平更低(P<0.05);与治疗前比较,两组治疗后中医症候各积分均降低,且与对照组比较,研究组治疗后中医症候各积分更低(P<0.05)。结论 加味二陈汤联合西药治疗老年社区获得性肺炎痰湿阻肺证的临床疗效确切,能够加快临床症状改善速度,减轻炎症反应,值得临床应用。
Objective To observe the clinical effect of modified Erchen decoction combined with western medicine in the treatment of elderly community-acquired pneumonia with syndrome of phlegm-damp obstructing lung.Methods A total of 64 elderly community-acquired pneumonia patients with syndrome of phlegm-damp obstructing lung who were treated in Leping Hospital of Traditional Chinese Medicine from January 2022 to March 2024 were selected as the research objects.According to the random number table method,they were divided into control group and study group,with 32 patients in each group.The control group was treated with conventional western medicine,and the study group was treated with modified Erchen decoction on the basis of the control group.The clinical efficacy,symptom elimination time(cough disappearance time,shortness of breath disappearance time,inflammation absorption time,body temperature recovery time,rale disappearance time),inflammatory cells [procalcitonin(PCT),neutrophil ratio(NEUT%),white blood cell count(WBC),C-reactive protein(CRP)] levels,and TCM syndromes(cough,fever,expectoration,chest tightness,fatigue) scores were compared between the two groups.Results Compared with 75.00% of the total effective rate of the control group,the total effective rate of the study group was higher,reaching 96.88%(P<0.05).Compared with the control group,the disappearance time of cough,the disappearance time of shortness of breath,the absorption time of inflammation,the recovery time of body temperature and the disappearance time of rales in the study group were shorter(P<0.05).Compared with before treatment,the levels of WBC,NEUT%,CRP and PCT in the two groups decreased after treatment,and compared with the control group,the levels of WBC,NEUT%,CRP and PCT in the study group were lower after treatment(P<0.05).Compared with before treatment,the scores of TCM symptoms in the two groups decreased after treatment,and compared with the control group,the scores of TCM symptoms in the study group were lower after treatment( P <0.05).Conclusion Modified Erchen decoction combined with western medicine has a definite clinical effect in the treatment of elderly community-acquired pneumonia patients with syndrome of phlegm-damp obstructing lung,which can accelerate the improvement of clinical symptoms and reduce inflammatory response.It is worthy of clinical application.
