摘要
目的通过分析全国中成药集中带量采购(以下简称“集采”)政策在某三甲综合医院的实施情况,以及中选药品说明书标注情况及相关政策联动情况,为集采制度完善及医院药事管理提供建议。方法对比分析南通市第一人民医院中选中成药集采前后6个月的使用量、销售金额、用药频度(DDDs)、日均费用(DDC)、集采完成进度及实际节约费用。统计中选药品说明书标注情况,梳理中选药品纳入国家基药目录、医保药品目录,入选中药大品种、中药保护品种情况。结果集采后,该院5个原有品种新增4个品规,原有品种DDC降低,使用量、DDDs与是否有新增规格与剂型存在关联;集采完成进度受协议分配量影响较大;集采实际节约费用共计116.04万元。中选中成药说明书存在表述模糊、标注缺失、执行标准不统一等问题。中选药品均纳入医保。脑安、心可舒为国家基本药物。5个品规为中药大品种,4个品规为中药保护品种。结论全国中成药集采政策实施切实降低了患者的用药负担,建议细化完善集采规则,规范药品说明书,加强药事管理,提高报量精准度,增强政策实施效果。
Objective To analyze the implementation of the national centralized procurement of Chinese patent medicine policy(hereinafter referred to as"collective procurement")in a third class general hospital,and the labeling of selected drugs and the linkage of related policies,and provide suggestions for the improvement of collection system and hospital pharmaceutical management.Methods The usage,defined daily doses(DDDs),defined daily cost(DDC),collection progress and actual cost savings of selected Chinese patent medicine were compared and analyzed in the First People's Hospital of Nantong City before and after 6 months of centralized volume-based procurement.Statistics on the labeling of selected drug instructions,sorting out the inclusion of selected drugs in the national essential drug list,medical insurance drug list,the selection of large varieties of traditional Chinese medicine,traditional Chinese medicine protection varieties.Results After centralized volume-based procurement,4 new product specifications were added to 5 original varieties in the hospital,and DDC of the original varieties decreased,and the usage amount and DDDs were related to whether there were new specifications and dosage forms.The completion progress of collective mining was greatly affected by the agreed distribution amount,and the actual saving cost of collective mining totaled 116.04 million yuan.There are some problems in the instructions of selected Chinese patent medicine,such as vague expression,missing marks and inconsistent execution standards.The selected drugs are included in medical insurance,and Naoan and Xinkeshu are national essential drugs.5 of them are large varieties of traditional Chinese medicine and 4 are protected varieties of traditional Chinese medicine.Conclusion The national collection of Chinese patent medicine has effectively reduced the drug burden of patients,and it is recommended to refine and improve the collection rules,further standardize drug instructions,strengthen pharmaceutical management,improve the accuracy of reporting,and enhance the effect of policy implementation.
