摘要
目的分析肺部肿物术中快速病理诊断与术后石蜡切片病理诊断不符合的原因,提出优化方案。方法收集厦门大学附属第一医院病理科2022年6月至2023年6月间收检的肺部肿物术中冰冻切片病理诊断的临床病理资料,对术中快速病理诊断与术后病理诊断不符合的病例进行分析。结果肺部肿物术中快速病理诊断与术后病理诊断符合率为99.38%(1611/1621),达到行业诊疗标准。有10例肺部肿物术中快速病理诊断与术后石蜡切片病理诊断不符合。其中术中诊断为恶性,术后诊断为良性3例;术中诊断为良性,术后诊断为恶性5例;术中与术后均诊断为恶性,疾病类型不符合2例。结论肺部肿物术中快速病理诊断与术后石蜡切片病理诊断不符合的原因包括诊断、取材、制片等因素,通过加强病理科医师培训、提升病理技师技术水平、优化病理工作流程、引进新技术等措施,可减少此类诊断不符合现象。
Objective To analyze the reasons for the inconsistency between rapid intraoperative pathological diagnosis and postoperative paraffin section pathological diagnosis of pulmonary masses,and to propose an optimal scheme.Methods The clinicopathological data of intraoperative frozen section pathological diagnosis of pulmonary masses collected from Department of Pathology,First Affiliated Hospital of Xiamen University from June 2022 to June 2023 were collected,and the cases with inconsistentintraoperative rapid pathological diagnosis and postoperative pathological diagnosis were analyzed.Results The coincidence rate of intraoperative rapid pathological diagnosis and postoperative pathological diagnosis was 99.38%(1611/1621),which reached the standard of diagnosis and treatment in the industry.The rapid intraoperative pathological diagnosis of lung masses was inconsistent with that of postoperative paraffin sections in 10 cases.Intraoperative and postoperative pathological diagnosis did not match:intraoperative diagnosis was malignant,postoperative diagnosis was benign in 3 cases;Intraoperative diagnosis was benign,postoperative diagnosis was malignant 5 cases;Both intraoperative and postoperative cases were diagnosed as malignant,and the disease type was inconsistent in 2 cases.Conclusion The reasons for the incompatibility between rapid pathologic diagnosis of pulmonary masses and postoperative pathological diagnosis of paraffin sections include:diagnosis,sampling and film production.Such inconsistencies can be reduced by strengthening the training of pathologists,improving the technical level of pathologists,optimizing the pathologic workflow and introducing new technologies.
