摘要
目的分析基于Rosswurm和Larrabee循证实践变革模式对预防ICU昏迷患者足下垂的护理干预措施优化效果。方法选取桂林医学院附属医院重症二病区2022年8月至2024年4月120例住院治疗的昏迷患者,按照随机数表法对其进行分组,分为试验组(基于Rosswurm和Larrabee循证实践变革模式的护理干预)和对照组(常规护理),分别比较两组患者的足下垂发生率、下肢肌肉萎缩情况[改良Ashworth量表(MAS)]、足部压力性损伤发生率,以评估基于循证实践的护理干预对预防足下垂的优化效果。结果经护理1周后,试验组临床足下垂率低于对照组患者(P<0.05);相较于对照组而言,治疗后试验组腓肠肌、比目鱼肌、股四头肌MAS评分均更低(P<0.05)。结论在临床预防ICU昏迷患者足下垂的临床护理中,基于Rosswurm和Larrabee循证实践变革模式护理不仅能够有效减少足下垂的发生,同时可有效缓解肌肉萎缩情况,改善预后。
Objective To analyze the optimization effect of the evidence-based practice change model based on Rosswurm and Larrabee.Methods A total of 120 patients with Comatose patient from Department of Critical Care Medicine,affiliated Hospital of Guilin Medical College from August 2022 to April 2024 were selected.The average group treatment according to the random number table method,divided into trial group(nursing intervention based on Rosswurm and Larrabee)and control group(usual care).The incidence of foot drop,lower limb muscle atrophy[modified Ashworth scale(MAS)]between the two groups.To evaluate the optimal effect of an evidence-based practice-based nursing intervention on the prevention of foot drop.Results After 1 week of clinically care,the clinical foot drop rate in the test group was lower than that in the control group(P<0.05).The gastrocnemius,soleus and quadriceps were lower in the control group(P<0.05).Conclusions In the clinical care to prevent foot drop in ICU coma patients,the nursing system based on Rosswurm and Larrabee can not only effectively reduce the occurrence of foot drop,but also effectively relieve the muscle atrophy and improve the prognosis.
