摘要
目的探究多层螺旋CT(multi-slice spiral CT,MSCT)在全髋关节置换术(total hip arthroplasty,THA)患者术后并发症中的应用价值。方法选取2021年6月-2023年6月在我院接受THA后因出现并发症行假体翻修术(prosthesis revision surgery,PRS)的60例患者,行假体翻修术前所有患者均接受过X线检查和MSCT检查。将PRS中病理检查结果作为金标准,评估X线检查和MSCT检查的诊断效能。结果手术病理检查结果显示:60例患者中以无菌性松动、骨溶解及感染占比最高,分别为51.67%、26.67%、30.00%。以手术病理检查结果为金标准,MSCT检查对THA术后无菌性松动、骨溶解、感染诊断的灵敏度、准确率均高于X线检查,对感染诊断的特异度高于X线检查(P<0.05),MSCT检查对THA术后各项并发症诊断的Youden指数均高于0.85。结论MSCT检查对THA患者术后各项并发症的诊断效能优于X线检查,具有较高的诊断价值。
Objective:To explore the application value of multi-slice spiral CT(MSCT)in postoperative complications of patients with total hip arthroplasty(THA).Methods:A total of 60 patients who underwent prothesis revision surgery(PRS)due to complications after undergoing THA in our hospital from June 2021 to June 2023 were selected.Before the PRS,all patients underwent X-ray and MSCT examinations.Make the pathological examination results in PRS as the gold standard to evaluate the diagnostic efficacy of X-ray and MSCT examinations.Results:The results of surgical pathological examination showed that among the 60 patients,aseptic loosening,bone resorption,and infection accounted for the highest proportion,which were 51.67%,26.67%,and 30.00%,respectively.Based on the results of surgical pathological examination as the gold standard,MSCT examination has higher sensitivity and accuracy in diagnosing aseptic loosening,bone resorption,and infection after THA surgery than X-ray examination,and higher specificity in diagnosing infection than X-ray examination(P<0.05).The Youden index of MSCT examination for diagnosing various complications after THA surgery was higher than 0.85.Conclusion:MSCT examination had better diagnostic efficacy than X-ray examination for various postoperative complications in THA patients,with higher diagnostic value.
