阶梯式功能训练对踝关节骨折儿童功能恢复 依从性及生活质量的影响 被引量:1

Effect of stepwise functional training on functional recovery,compliance,and quality of life in children with ankle fracture

摘要 目的探讨阶梯式功能训练对踝关节骨折儿童功能恢复、依从性及生活质量的影响。方法选取2019年1月—2022年11月在温州市中西医结合医院和温州医科大学附属第二医院住院治疗的100例踝关节骨折儿童,随机分成常规组(常规康复训练)和阶梯组(阶梯式功能训练),每组50例患者。比较两组患者踝关节功能评分、踝关节活动度、生活质量评分及依从性。结果训练前,两组患者踝关节功能评分比较差异均无统计学意义(均P>0.05);训练后,两组患者踝关节功能评分均升高,阶梯组患者踝关节功能评分[(67.43±5.82)分]、踝与足功能评分[(79.45±8.13)分]均高于常规组[(64.92±5.70)分、(75.61±8.45)分],差异均有统计学意义(t=2.179、2.316,均P<0.05)。训练前,两组患者踝关节活动度比较差异均无统计学意义(均P>0.05);训练后,两组患者踝关节活动度均增加,阶梯组患者跖屈活动度[(35.97±4.32)°]和背伸活动度[(21.07±3.09)°]均高于常规组[(33.86±4.25)°、(19.63±2.85)°],差异均有统计学意义(t=2.462、2.422,均P<0.05)。训练前,两组患者生活质量评分比较差异均无统计学意义(均P>0.05);训练后,两组患者生活质量评分均升高,阶梯组患者的身体评分[(89.74±5.26)分]、精神评分[(88.61±5.94)分]、社交评分[(89.93±5.62)分]、物质环境评分[(92.01±5.38)分]及综合评分[(90.14±5.17)分]均高于常规组[(87.46±5.37)分、(85.97±6.02)分、(87.62±5.41)分、(89.72±5.41)分、(88.03±4.59)分],差异均有统计学意义(t=2.145、2.207、2.094、2.122、2.158,均P<0.05)。阶梯组患者的总依从性(96.00%)明显高于常规组(82.00%),差异有统计学意义(χ^(2)=5.005,P<0.05)。结论阶梯式功能训练在踝关节骨折儿童的康复过程中展现出显著优势,通过分阶段、个性化的训练方案,有效促进了患者的功能恢复,提高了关节活动度,显著增强了患者的训练依从性,提高了其对康复训练的积极性和参与度,这些积极变化共同作用,提高了患者的生活质量,阶梯式功能训练是踝关节骨折儿童康复中不可或缺的重要手段。 Objective To explore the effect of stepwise functional training on functional recovery,compliance,and quality of life in children with ankle fracture.Methods From January 2019 to November 2022,100 patients with ankle fracture hospitalized in Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine and the Second Affiliated Hospital of Wenzhou Medical University were selected and randomly divided into routine group(routine rehabilitation training)and ladder group(stepwise functional training),50 patients in each group.The ankle function score,ankle mobility,compliance,and quality of life were compared between the two groups.Results Before training,there was no statistically significant difference in ankle function score between the two groups(P>0.05).After training,ankle function scores of the two groups increased,ankle function score[(67.43±5.82)points]and ankle and foot function score[(79.45±8.13)points]in ladder group were significantly higher than those in routine group[(64.92±5.70)points,(75.61±8.45)points],there were statistically significant differences(t=2.179,2.316,P<0.05).Before training,there was no statistically significant difference in ankle joint activity between the two groups(all P>0.05).After training,ankle joint activity of the two groups increased,plantar flexion activity[(35.97±4.32)°]and dorsal extension activity[(21.07±3.09)°]in ladder group were significantly higher than those in routine group[(33.86±4.25)°,(19.63±2.85)°],there were statistically significant differences in ankle joint activity between the two groups(t=2.462,2.422,all P<0.05).Before training,there was no statistically significant difference in quality of life between the two groups(P>0.05).After training,the quality of life of the two groups increased,physical score[(89.74±5.26)points],mental score[(88.61±5.94)points],social score[(89.93±5.62)points],material environment score[(92.01±5.38)points],and comprehensive score[(90.14±5.17)points]in ladder group were higher than those in routine group[(87.46±5.37)points,(85.97±6.02)points,(87.62±5.41)points,(89.72±5.41)points,(88.03±4.59)points],there were statistically significant differences(t=2.145,2.207,2.094,2.122,2.158,all P<0.05).The total compliance of patients in ladder group(96.00%)was higher than that in routine group(82.00%),there was statistically significant difference(χ^(2)=5.005,P<0.05).Conclusion Stepwise functional training shows significant advantages in rehabilitation process of children with ankle fracture.Through phased and personalized training programs,it effectively promotes functional recovery of the children and the improvement of joint mobility.It significantly enhances the training compliance of the children and improves their enthusiasm and participation in rehabilitation training.These positive changes work together on the quality of life of children,which is an indispensable and important method in rehabilitation of children with ankle fracture.
