摘要
目的本文通过分析按病种分值付费(DIP)对5个特定病种超长住院患者住院日和住院费用的影响,为提高医院医疗服务质量提供依据。方法提取宿迁市第一人民医院DIP实施前(2020年1月1日至12月31日)和DIP实施后(2021年1月1日至12月31日)主要病种例数排名前五位的超长住院患者的病例数据,分别为77例和49例,统计两组数据的住院日和住院费用情况,并进行分析比较。结果实施DIP后,宿迁市第一人民医院5个特定病种超长住院患者的平均住院日和平均住院费用均有所下降,差异有统计学意义(P<0.05)。结论因本研究的样本医院同期并没有出台其他政策措施,故DIP政策的实施有利于缩短超长住院患者住院日,促进住院费用结构优化,进而激励医院提高诊疗服务水平,达到提高医疗保障效率的作用。
Objective To analyze the effect of diagnosis-intervention packet(DIP)on the length of stay and hospitalization costs of prolonged stay patients with five specific diseases to provide a basis for enhancing the quality of medical services at the hospital.Methods Case data were extracted from prolonged stay patients with major diseases ranking top five in terms of number of cases at Suqian First Hospital prior to the implementation of DIP(from January 1 to December 31,2020)and after the implementation of DIP(from January 1 to December 31,2021),with 77 patients and 49 patients identified,respectively.Length of stay and hospitalization costs for both groups were counted,analyzed and compared.Results Following the implementation of DIP,the average length of stay and hospitalization costs were decreased for prolonged stay patients with the five specific diseases at Suqian First Hospital,with statistically significant differences(P<0.05).Conclusion Given that no other policy measures were introduced in the sample hospital during the same period,it can be concluded that the implementation of the DIP policy can result in a reduction in the length of stay for patients with prolonged stay.It also can promote the optimization of hospitalization cost structures and motivate hospitals to enhance the quality of diagnostic and treatment services,ultimately improving the efficiency of medical insurance.
