摘要
目的探讨食管癌患者症状群的构成,构建症状网络,探索核心症状,为制订症状管理方案提供参考。方法便利抽样法选取255例食管癌患者进行调查,采用一般资料调查表、中文版欧洲癌症患者生活质量调查量表和食管癌患者特异性生活质量调查量表评估其症状发生情况,通过探索性因子分析提取症状群,运用R语言构建症状网络,分析中心性指标。结果食管癌患者共提取6个症状群,分别是疲乏-食欲相关症状群,进食困难相关症状群,呼吸相关症状群,腹泻-反流相关症状群,言语相关症状群,疼痛相关症状群。其中疲乏、食欲减退、疼痛、恶心呕吐和吞咽困难是核心症状。疲乏(rs=4.45270,rc=0.01261)和食欲减退(rs=4.31811,rc=0.01257)的强度和紧密度较高。结论医护人员应关注患者的症状变化,通过对症状群及核心症状进行全程干预,将症状群与网络中心性指标相结合,制订精准的症状管理方案,改善患者治疗期间的生活质量。
Objective To explore the composition of symptom clusters in esophageal cancer patients,construct a symptom network,explore core symptoms,and provide reference for the development of symptom management programs.Methods Convenient sampling method was used to select 255 esophageal cancer patients for investigation,and their symptom occurrence was assessed by using the general information questionnaire,the Chinese version of the 30-item European Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQ-C30)and the 18-item Quality of Life Questionnaire,Esophageal Cancer-Specific(QLQ-OES 18);symptom clusters were extracted by exploratory factor analysis,and symptom networks were constructed and the centrality indexes were analyzed by using the language of R.Results There were six symptom clusters extracted from esophageal cancer patients,which were fatigue appetite-related symptom cluster,eating difficulty-related symptom cluster,respiration-related symptom cluster,diarrhea reflux-related symptom cluster,speech-related symptom cluster,and pain-related symptom cluster.Fatigue,decreased appetite,pain,nausea,vomiting,and difficulty swallowing were the core symptoms.The intensity and tightness of fatigue(r s=4.45270,r c=0.01261)and decreased appetite(r s=4.31811,r c=0.01257)were relatively high.Conclusion Healthcare professionals should pay attention to the changes in patients′symptoms,intervene throughout the entire process of symptom clusters and core symptoms,combine symptom clusters with network centrality indicators,and develop precise symptom management plans to improve patients′quality of life during treatment.
