摘要
目的探讨预冲式留置针进针法对外周浅静脉评级为4级的老年患者穿刺的临床应用效果。方法选取84例因手术需要输液且血管评级均为4级的老年患者,年龄为70~90岁,随机分为试验组42例和对照组42例,试验组采用预冲式留置针进针法,对照组采用传统的穿刺进针法。比较两组患者留置针一针穿刺的成功率、穿刺时的疼痛程度及患者护理满意度的差异。结果试验组患者一次性穿刺成功率(88.1%)较对照组(66.7%)明显升高(P=0.019);试验组患者无痛15例、轻度疼痛14例、中度疼痛8例、重度疼痛5例,对照组患者无痛6例、轻度疼痛8例、中度疼痛18例、重度疼痛10例,因此试验组疼痛程度明显低于对照组(P=0.012);试验组患者护理满意度较对照组明显升高(P=0.021)。结论对于手术后外周浅静脉评估为4级的老年患者,相比于传统静脉穿刺进针法,预冲式留置针进针法不仅一次性穿刺成功率更高,还可以显著减少患者疼痛不适感,提高服务满意度,避免医疗矛盾,对临床护理技术提升有推广价值。
Objective To investigate the clinical application of the pre-flush indwelling needle insertion method for puncture in elderly patients with a peripheral superficial vein rating of level 4.Methods 84 elderly patients,aged 70~90,who required intravenous infusion for surgical procedures and had a peripheral superficial vein rating of level 4,were randomly divided into an experimental group(42 cases)and a control group(42 cases).The experimental group adopted the pre-flush indwelling needle insertion method,which involved withdrawing the needle core first,then opening the fluid stopcock,and slowly and smoothly inserting the needle tube in a pre-flush manner.The control group received the traditional puncture and needle insertion method.The success rate of one-time puncture with the indwelling needle,the degree of pain during puncture,and patient satisfaction were compared between the two groups.Results The success rate of one-time puncture in the experimental group(88.1%)was significantly higher than that in the control group(66.7%)(P=0.019).In the experimental group,there were no pain(15 cases),mild pain(14 cases),moderate pain(8 cases),and severe pain(5 cases),while in the control group,there were no pain(6 cases),mild pain(18 cases),moderate pain(18 cases),severe pain(10 cases).Thus,the pain level in the experimental group was significantly lower than that in the control group(P=0.012).Patient satisfaction with nursing care was significantly higher in the experimental group compared to the control group(P=0.021).Conclusion For elderly surgical patients with a peripheral superficial vein rating of level 4,the pre-flush indwelling needle insertion method not only significantly improves the success rate of one-time puncture compared to the traditional method,but also significantly reduces patient discomfort and pain,improves service satisfaction,and avoided medical contradiction.It has the value of promotion for the improvement of clinical nursing technology.
