摘要
银屑病是一种免疫介导的慢性、复发性、炎症性、系统性疾病,代谢相关脂肪性肝病是银屑病的常见共病之一。本文浅析银屑病合并代谢相关脂肪性肝病的中医病因病机。两病共病病位在血,涉及肝、脾、肾三脏,其病机演变从早期的湿热到中期的痰热,再到后期的瘀热,其中血热是推动这一演变过程的关键。脏腑辨证方面,由血热损伤肝络而起,病情的虚实会相互转化,并在转化过程中相互加剧。两病共病治疗时应辨明症情阶段,重视祛除无形之热和有形之湿痰瘀,并注意顾护肝脾气阴。
Psoriasis is an immune-mediated chronic,relapsing,inflammatory,systemic disease,and metabolic dysfunction-associated fatty liver disease(MAFLD)is one of the most common co-morbidities of psoriasis.This paper interpreted the traditional Chinese medicine(TCM)etiology of psoriasis complicated with MAFLD.The two co-morbidities are located in the blood,involving the liver,spleen and kidney,and their TCM pathogenesis has evolved through three stages,from dampness-heat in the early stage to phlegm-heat in the middle stage and then to stasis-heat in the late stage,with blood-heat being the key to driving this evolutionary process.In TCM syndrome differentiation system of viscera,blood-heat damage to the liver complex and the beginning of the disease,the condition of the deficiency and reality of the existence of mutual transformation exacerbated.In the treatment of co-morbidities,the stage of the disease should be identified,emphasis should be placed on eliminating invisible heat and visible dampness,phlegm and stasis,and attention should be paid to the care of Qi and Yin.
