摘要
目的 研究经脐单孔腹腔镜前哨淋巴结活检(SLNB)加双附件切除术治疗早期子宫内膜癌(EC)的临床效果。方法 回顾性分析152例EC患者临床资料,依据手术方法的不同将其分为对照组(76例)与观察组(76例)。观察组采用经脐单孔腹腔镜SLNB加双附件切除术治疗,对照组采用传统腹腔镜SLNB加双附件切除术治疗。比较2组围手术期指标、疼痛因子[5-羟色胺(5-HT)、多巴胺(DA)]、肿瘤因子[癌抗原125(CA125)、癌抗原199(CA199)]及并发症。结果 观察组住院时间短于对照组(P<0.05);术后2组5-HT、DA均高于术前,但观察组低于对照组,差异有统计学意义(P<0.05);2组术中出血量、手术时间及术后CA125、CA199水平、并发症总发生率比较,差异无统计学意义(P>0.05)。结论 经脐单孔腹腔镜SLNB加双附件切除术治疗早期EC的效果及安全性与传统腹腔镜手术相当,但单孔腹腔镜手术治疗早期EC在减轻患者术后疼痛及加快术后恢复方面更具优势。
Objective To study the clinical effect of laparoscopic sentinel lymph node biopsy(SLNB)plus double adnexectomy for early endometrial carcinoma(EC).Methods The clinical data of 152 EC patients were retrospectively analyzed and divided into control group(76)and observation group(76)according to different surgical methods.The observation group was treated with transumbilical single port laparoscopic SLNB plus double accessory resection,and the control group was treated with conventional laparoscopic SLNB plus double accessory resection.Perioperative indicators,pain factors[serotonin(5-HT),dopamine(DA)],tumor factors[cancer antigen 125(CA125),cancer antigen 199(CA199)]and complications were compared.Results The hospital stay in the observation group was shorter than that of the control group(P<0.05),5-HT and DA were higher than preoperative,but the observation group was statistically significant(P<0.05),and intraoperative bleeding in the 2 groups,postoperative CA125,CA199 levels,and overall incidence of complications(P>0.05).Conclusion The effect and safety of laparoscopic SLNB and double accessory resection are comparable to that of traditional laparoscopic surgery,but it has more advantages in reducing postoperative pain and accelerating postoperative recovery.
