摘要
目的探讨二维超声检查联合煊流成像技术诊断胎儿脐带真结的临床价值。方法选取我院行产前超声检查的孕妇10038例,应用常规超声检查(二维超声检查+彩色多普勒超声检查)和二维超声检查联合煊流成像技术判断胎儿脐带有无真结形成。根据出生后有无脐带真结分为脐带有真结34例作为观察组和脐带无真结100例作为对照组。计算两种检查方法诊断脐带真结的敏感性、特异性、准确率、阳性预测值、阴性预测值。比较二维超声检查联合煊流成像技术诊断脐带真结是否优于常规超声检查。结果诊断脐带真结敏感性、特异性、准确率、阳性预测值、阴性预测值分别为:二维超声检查联合煊流成像技术(94.12%、100%、98.51%、100%、98.04%),常规超声检查(70.59%、94.00%、88.06%、80.00%、90.38%),差异均有统计学意义(P均<0.05)。结论二维超声检查联合煊流成像技术在诊断胎儿脐带真结的敏感性、特异性、准确率、阳性预测值、阴性预测值方面均优于常规超声检查,对于临床医师密切监测胎儿生长发育、决定分娩时机和分娩方式,减少妊娠不良结局具有重要临床意义。
Objective To investigate the clinical value of two-dimensional ultrasound combined with HD live flow in the diagnosis of true umbilical cord knot.Methods 10038 pregnant women who underwent prenatal ultrasonography in the Department of Medical Ultrasound of Zaozhuang Maternal and Child Health Hospital were selected.Conventional ultrasonography(twodimensional ultrasonography and color Doppler ultrasonography)and two-dimensional ultrasonography combined HD live flow were used to determine whether the fetal umbilical cord was formed.According to the presence or absence of true umbilical cord knot after birth,the group was further divided into the group with true umbilical cord knot and the group without true umbilical cord knot.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the two methods for the diagnosis of true umbilical cord knot were calculated to compare whether two-dimensional ultrasound combined with HD live flow was better than conventional ultrasound in diagnosing true umbilical cord knot.Results The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the diagnosis of true umbilical cord knot were as follows:the yield by two-dimensional ultrasound combined with HD Live flow was 94.12%,100%,98.51%,100%,and 98.04%,while,the yield with conventional ultrasound examination reached 70.59%,94.00%,88.06%,80.00%,90.38%,respectively,all the P values were less than 0.05.Conclusion The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the diagnosis of true umbilical cord knot by two-dimensional ultrasound combined with HD Live flow are superior to those by conventional ultrasonography.It has important clinical significance for clinicians to closely monitor fetal growth and development as well as to decide the timing and mode of delivery,thereby reducing adverse pregnancy outcomes.
