摘要
目的:分析640层CT双低技术一站式扫描在急性缺血性脑卒中(AIS)诊断中的应用。方法:分析2023年1月至2023年9月60例急性AIS患者有关资料,所有患者均采用640层CT双低技术一站式扫描,对有关指标进行分析。结果:颅脑灌注有效辐射剂量以及头颈部CTA及颅脑CTP总辐射剂量分别为(2.26±0.09)mSv、(3.22±0.16)mSv。60例患者均获得清晰图像用于临床诊断,椎-基底动脉系统狭窄有63处,轻度、中度、重度狭窄分别为36处、18处、9处;颈动脉129处,轻度、中度、重度狭窄分别为78处、29处、22处。不同狭窄程度患者的rCBF、rCBV、MTT、TTP及TMax平均水平比较,差异有统计学意义(F=12.619、16.721、3.211、11.515、11.323,P<0.05)。rCBF、rCBV、MTT、TTP和TMax评估AIS的ROC的AUC均>0.5,且联合评估的AUC高于上述各指标单独评估(P<0.05)。结论:640层CT双低技术一站式扫描能够同时发现血管狭窄、灌注异常,rCBF、rCBV、MTT、TTP和TMax联合诊断结果,具有比较高的敏感性,因此具有一定的临床价值,可用于指导患者的临床治疗方向。
Objective:Research on the application of 640-slice CT dual-low technique in one-stop scanning for the diagnosis of acute ischemic stroke(AIS).Methods:Analyze the relevant data of 60 patients with AIS from January 2023 to September 2023.All patients were scanned using 640-slice CT dual-low technique in one-stop scanning,and the relevant indicators were analyzed.Results:The effective radiation dose of cerebral perfusion and the total radiation dose of head and neck CTA and cranial CTP were(2.26±0.09)mSv and(3.22±0.16)mSv,respectively.All 60 patients obtained clear images for clinical diagnosis.There were 63 stenosis sites in the vertebral basilar artery system,with 36 mild,18 moderate,and 9 severe stenosis sites,respectively.There are 129 carotid arteries,with 78 mild,29 moderate,and 22 severe stenosis,respectively.The average levels of rCBF,rCBV,MTT,TTP,and TMax in patients with different degrees of stenosis were compared,and the differences were statistically significant(F=12.619,16.721,3.211,11.515,11.323,P<0.05).The AUC of ROC for AIS evaluated by rCBF,rCBV,MTT,TTP,and TMax were all greater than 0.5,and the AUC of the combined evaluation was higher than that of the individual evaluations of the above indicators(P<0.05).Conclusion:The 640-slice CT dual-low dose technology one-stop scanning can simultaneously detect vascular stenosis and perfusion abnormalities.The combined diagnosis of rCBF,rCBV,MTT,TTP,and TMax has high sensitivity and certain clinical value,which can be used to guide the clinical treatment direction of patients.
