卵巢附件O-RADS分类联合超声多普勒血流信号在卵巢附件良恶性肿瘤中的应用价值 被引量:2

The Application Value of Ovarian Adnexal O-Rads Classification Combined with Ultrasound Doppler Blood Flow Signals in Benign and Malignant Ovarian Adnexal Tumors

摘要 目的探讨卵巢附件影像报告和数据系统(O-RADS)分类联合超声多普勒血流信号在卵巢附件良恶性肿瘤中的应用价值。方法选取2022年10月-2024年6月在郑州市妇幼保健院行超声检查且经术后病理证实的卵巢附件肿物患者110例,采用O-RADS进行分类,并行超声多普勒检查,采用ROC曲线及决策树模型综合评估O-RADS分类、超声多普勒血流信号对卵巢附件良恶性肿瘤的鉴别价值。结果卵巢附件良性肿瘤76例纳入良性组,34例恶性肿瘤纳入恶性组,良性组O-RADS分类2类率、4类率与恶性组比较,差异无统计学意义(P>0.05),良性组O-RADS分类3类率(36.84%)高于恶性组(2.94%),差异有统计学意义(χ^(2)=13.907,P<0.05),良性组O-RADS分类5类率(14.47%)低于恶性组(67.65%),差异有统计学意义(χ^(2)=31.101,P<0.05)。ROC曲线分析得出,O-RADS分类、超声多普勒血流信号、二者联合鉴别卵巢附件良恶性的AUC分别为0.831、0.828、0.943,敏感度分别为67.6%、85.3%、82.4%,特异度分别为85.5%、80.3%、89.5%。以卵巢附件良恶性为因变量,以O-RADS分类、超声多普勒血流信号、二者联合为自变量分别构建卵巢附件良恶性决策树鉴别模型,误判率分别为20.0%、18.2%、13.6%,预测准确率分别为80.0%、81.8%、86.4%。结论卵巢附件O-RADS分类联合超声多普勒血流信号应用于卵巢附件良恶性肿瘤的鉴别中,具有较高的鉴别价值。 Objective To explore the application value of ovarian adnexal imaging report and data system(O-RADS)classification combined with ultrasound Doppler blood flow signals in the diagnosis of benign and malignant ovarian adnexal tumors.Methods 110 patients with ovarian adnexal masses who underwent ultrasound examination in our hospital from October 2022 to June 2024 and were confirmed by postoperative pathology were selected.O-RADS was used for classification,and ultrasound Doppler examination was performed.ROC curves and decision tree models were used to comprehensively evaluate the differential value of O-RADS classification and ultrasound Doppler blood flow signals in the diagnosis of benign and malignant ovarian adnexal tumors.Result 76 cases of benign ovarian adnexal tumors were included in the benign group,and 34 cases of malignant tumors were included in the malignant group.The O-RADS classification rate of Class 2 and Class 4 in the benign group was not statistically significant compared to the malignant group(P>0.05).The O-RADS classification rate of Class 3 in the benign group was 36.84%,which was higher than 2.94%in the malignant group(χ^(2)=13.907,P<0.05).The O-RADS classification rate of Class 4 in the benign group was 14.47%,which was lower than 67.65%in the malignant group(P<0.05).The detection rate of ultrasound Doppler blood flow signal in the benign group was 19.74%,which was lower than 85.29%in the malignant group(χ^(2)=31.101,P<0.05).The ROC curve analysis showed that the AUC for O-RADS classification,ultrasound Doppler blood flow signal,and combined differentiation of benign and malignant ovarian appendages were 0.831,0.828,and 0.943,respectively,with sensitivity of 67.6%,85.3%,and 82.4%,and specificity of 85.5%,80.3%,and 89.5%,respectively.A PFO ovarian accessory benign and malignant decision tree discrimination model was constructed using O-RADS classification,ultrasound Doppler blood flow signal,and their combination as independent variables,with benign and malignant ovarian accessory as the dependent variable.The misjudgment rates were 20.0%,18.2%,and 13.6%,respectively,and the prediction accuracy was 80.0%,81.8%,and 86.4%,respectively.Conclusion The combination of ovarian adnexal O-RADS classification and ultrasound Doppler blood flow signals has high diagnostic value in the differentiation of benign and malignant ovarian adnexal tumors.
