肩部触发点体外冲击波联合核心肌群运动训练对肩袖损伤患者肩关节功能及肌腱弹性的影响 被引量:3

摘要 目的分析肩部触发点体外冲击波联合肩袖损伤核心肌群运动训练对肩袖损伤患者肩关节功能和肌腱弹性的影响。方法将东南大学医学院附属南京同仁医院2021年7月至2024年10月收治的102例肩袖损伤患者随机分成2组,对照组接受肩部触发点体外冲击波联合常规训练,观察组给予肩部触发点体外冲击波联合核心肌群运动训练,治疗12周,评价比较临床疗效、肩关节功能、肩关节活动度以及肌腱弹性。结果观察组临床治疗总有效率更高(P<0.05)。干预后组间比较,观察组Constant-Murley各项目得分均更高(P<0.05)。干预后观察组患者肩关节外旋角度、肩关节外展角度及肩关节前屈角度均高于对照组(P<0.05)。干预后观察组冈下肌、冈上肌、小圆肌及肩胛下肌肩关节肌腱弹性低于对照组(P<0.05)。结论肩袖损伤患者接受肩部触发点体外冲击波治疗并进行核心肌群运动训练对可有效改善肩关节功能和肌腱弹性。 Objective To analyze the effects of shoulder trigger point extracorporeal shock wave combined with core muscle group exercise training on shoulder joint function and tendon elasticity in patients with rotator cuff injury.Methods 102 patients with rotator cuff injuries admitted to Nanjing Tongren Hospital Affiliated to Southeast University School of Medicine from July 2021 to October 2024 were randomly divided into two groups.The control group received shoulder trigger point extracorporeal shock wave combined with conventional training,while the observation group received shoulder trigger point extracorporeal shock wave combined with core muscle group exercise training for 12 weeks.Clinical efficacy,shoulder joint function,shoulder joint range of motion,and tendon elasticity were evaluated and compared.Results The total effective rate of treatment in the observation group was significantly higher(P<0.05).After intervention,the observation group showed higher scores in all Constant-Murley items compared to the control group(P<0.05).After intervention,the shoulder joint external rotation angle,shoulder joint abduction angle,and shoulder joint flexion angle of the observation group were all higher than those of the control group(P<0.05).After intervention,the elasticity of the shoulder joint tendons of the infraspinatus,supraspinatus,teres minor,and infrascapular muscles in the observation group was lower than that in the control group(P<0.05).Conclusion Patients with rotator cuff injuries receiving treatment at the shoulder trigger point and core muscle group exercise training can effectively improve shoulder joint function and tendon elasticity.
