基于骨健康探究颈椎病中医药防治策略与研究展望

Investigating traditional Chinese medicine strategies and prospects for prevention and treatment of cervical spondylosis based on bone health

摘要 颈椎病作为脊柱退行性疾病的典型疾患,具有发病率高、复发率高、致残率高的特点,并随着学习、工作、生活方式的改变,呈职业化、年轻化趋势,已成为严重影响国民健康的疾病之一。随着对颈椎病的发病机理、疾病进展、临床研究和预后转归的深化研究,该病的诊疗指南和专家共识得到进一步完善。普遍认为,正骨手法、中药、功能锻炼等中医疗法是治疗该病的有效手段,但其以经验为主、科学证据不足、疗效难以评价、传承教学方法落后等问题制约了中医发展和推广。如何科学地阐述中医药防治颈椎病的作用与优势,是骨伤科现代化发展的重要突破口,符合“健康中国”的战略需求。朱立国院士指出,骨健康是指导中医药防治颈椎病的重要方针,提出青少年人群从筋论治保发育、中青年人群筋骨并重调形态、老年人群强筋健骨稳功能的防治策略,并建议从“筋骨平衡”角度融合分子生物、生物力学、神经电生理、影像组学、人工智能、自动化控制等先进技术开展颈椎病的中医药传承与创新研究工作,探索颈椎病的中医药作用机理与优势。 Cervical spondylosis,as a typical degenerative disease of the spine,is characterized by high incidence,high recurrence rate,and high disability rate.With changes of study,work and lifestyle,it has become increasingly prevalent among younger and occupational groups,thus emerging as one of the serious health concerns affecting the populace.As research into the etiology,progression,clinical studies,and prognosis of cervical spondylosis deepens,the diagnostic and therapeutic guidelines and expert consensus for this condition have been further refined.It is widely recognized that traditional Chinese medical treatments such as orthopedic manipulation,herbal medicine,and functional exercises are effective means of managing this condition.However,challenges such as reliance on empirical knowledge,insufficient scientific evidence,difficulty in assessing efficacy,and outdated teaching methods hinder the development and dissemination of traditional Chinese medicine.Articulating the role and advantages of traditional Chinese medicine in preventing and treating cervical spondylosis scientifically is a crucial breakthrough point for the modernization of orthopedics and aligns with the strategic imperative of'Healthy China'.Academician ZHU Liguo pointed out that bone health is a key principle guiding the prevention and treatment of cervical spondylosis with traditional Chinese medicine.He proposed preventive and therapeutic strategies for different age groups:focusing on tendon treatment to promote development in adolescents,emphasizing both tendon and bone health to adjust morphology in middle-aged patients,and advocating for tendon strengthening and bone health maintenance to stabilize functionality in elderly patients.He also suggested integrating advanced technologies such as molecular biology,biomechanics,neurophysiology,imaging omics,artificial intelligence,and automation from the perspective of‘tendon-bone balance'to conduct innovative research on the inheritance and innovation of traditional Chinese medicine for cervical spondylosis,thereby exploring the mechanisms and advantages of traditional Chinese medicine in managing this condition.
