摘要
目的探讨急性心肌梗死患者的血清生长分化因子-15(GDF-15)、脂蛋白相关磷脂酶A_(2)(Lp-PLA_(2))、心型脂肪酸结合蛋白(H-FABP)水平与病情程度的相关性。方法选取2021年6月至2023年6月我科收治的100例急性心肌梗死患者作为观察组[根据冠脉病变支数分为单支(25例)、双支(40例)、三支(35例),并根据Killip分级分为Ⅰ级(34例)、Ⅱ级(25例)、Ⅲ级(29例)以及Ⅳ级(12例)];同期选取100名进行体检的健康者作为对照组。检测并比较各组的GDF-15、Lp-PLA_(2)、H-FABP水平,分析其与病情程度的相关性。结果观察组的血清GDF-15、Lp-PLA_(2)及H-FABP水平显著高于对照组(P<0.05)。双支、三支冠脉病变急性心肌梗死患者的血清GDF-15、Lp-PLA_(2)、H-FABP水平显著高于单支冠脉病变急性心肌梗死患者(P<0.05)。Ⅱ级、Ⅲ级、Ⅳ级急性心肌梗死患者的血清GDF-15、Lp-PLA_(2)、H-FABP水平显著高于Ⅰ级急性心肌梗死患者(P<0.05)。血清GDF-15、Lp-PLA_(2)、H-FABP与急性心肌梗死冠脉病变支数、Killip分级呈正相关(P<0.05)。结论血清GDF-15、Lp-PLA_(2)、H-FABP水平变化与急性心肌梗死患者病情程度密切相关,表达水平越高提示病情程度越严重,可作为急性心肌梗死病情程度的评估指标。
Objective To investigate the correlation between serum growth differentiation factor-15(GDF-15),lipoprotein-associated phospholipase A_(2)(Lp-PLA_(2)),heart-type fatty acid-binding protein(H-FABP)levels and severity of acute myocardial infarction in patients.Methods A total of 100 patients with acute myocardial infarction admitted in our department from June 2021 to June 2023 were selected as the observation group[according to the number of coronary artery lesions,the patients were divided into single-vessel(25 cases),double-vessel(40 cases)and triple-vessel(35 cases);and according to Killip classification,the patients were divided into gradeⅠ(34 cases),gradeⅡ(25 cases),gradeⅢ(29 cases)and gradeⅣ(12 cases)];at the same time,100 healthy people underwent physical examiniation were selected as the control group.The levels of GDF-15,Lp-PLA_(2) and H-FABP in each group were detected and compared,and their correlation with the severity of the disease was analyzed.Results The levels of serum GDF-15,Lp-PLA_(2) and H-FABP in the observation group were significantly higher than those in the control group(P<0.05).The levels of serum GDF-15,Lp-PLA_(2) and H-FABP in patients with acute myocardial infarction with double-vessel and triple-vessel coronary artery disease were significantly higher than those in patients with acute myocardial infarction with single-vessel coronary artery disease(P<0.05).The levels of serum GDF-15,Lp-PLA_(2) and H-FABP in patients with gradeⅡ,gradeⅢand gradeⅣacute myocardial infarction were significantly higher than those in patients with gradeⅠacute myocardial infarction(P<0.05).The serum GDF-15,Lp-PLA_(2) and H-FABP were positively correlated with the number of coronary artery lesions and Killip grade in acute myocardial infarction(P<0.05).Conclusion The changes of serum GDF-15,Lp-PLA_(2) and H-FABP levels are closely related to the severity of acute myocardial infarction.The higher the expression level is,the more serious the disease is,which can be used as an evaluation index for the severity of acute myocardial infarction.
