摘要
目的探讨不同胆囊癌发生部位的手术患者近期预后差异,分析预后的影响因素。方法选取80例胆囊癌手术患者,根据肿瘤部位的不同分为颈部组22例和底体部组58例。采用Kaplan-Meier法绘制生存曲线,比较两组患者的近期预后。采用Cox比例风险回归模型进行近期预后影响因素分析。结果底体部组和颈部组胆囊癌手术患者R_(0)切除率、TNM分期、N分期比较,差异均有统计学意义(P<0.05)。底体部组患者生存情况明显优于颈部组患者,差异有统计学意义(P<0.01)。多因素Cox分析结果显示,TNM分期、N分期、肿瘤部位均为胆囊癌手术患者近期预后的影响因素(P<0.05)。结论不同胆囊癌发生部位的手术患者R_(0)切除率、TNM分期、N分期有差异,胆囊颈部肿瘤手术患者近期预后更差,肿瘤部位、TNM分期、N分期均为胆囊癌手术患者近期预后的影响因素。
Objective To investigate the difference of short-term prognosis in patients undergoing surgery at different sites of gallbladder cancer,and to analyze the influencing factors of prognosis.Method A total of 80 patients with gallbladder cancer were selected and divided into neck group(22 cases)and base group(58 cases)according to different tumor sites.Kaplan-Meier was used to draw the survival curve,and the short-term prognosis of the two groups was compared.Cox proportional hazards regression model was used to analyze the influencing factors of the short-term prognosis.Result There were significant differences in R_(0)resection rate,TNM stage and N stage between base group and neck group(P<0.05).The survival of base group was significantly better than that of the neck group,and the difference was statistically significant(P<0.01).Multirariate Cox analysis showed that TNM stage,N stage,and tumor site were influencing factors of the short-term prognosis of patients with gallbladder cancer undergoing surgery(P<0.05).Conclusion The R_(0)resection rate,TNM stage and N stage are different in patients undergoing surgery at different sites of gallbladder cancer.The short-term prognosis of patients with gallbladder neck tumor is worse.Tumor site,TNM stage,N stage are influencing factors of the short-term prognosis of patients with gallbladder cancer.
