摘要
帕金森病(PD)是目前全球常见的神经系统退行性疾病之一,已成为近年的研究热点。西医常规多巴胺替代疗法存在运动波动、视觉幻觉等副作用使患者依从性不佳。PD属中医学“颤证”范畴,俞教授以阴火理论为指导,系统总结该思想指导下各期PD的病机及辨证论治,提出阴火燔灼为PD核心病机,初期真阴不足、阴火伏匿,当滋补肾阴,以三才封髓丹加减处之;中期心火助热、阴火炽盛,当清心涤热,以左归丸加减处之;晚期痰瘀痼结、阴火渐衰,当化痰逐瘀,以金水六君煎合桃红四物汤加减处之。本文基于阴火衍变视角梳理PD病机演变,并系统归纳不同分期PD治疗方案,以期为临床相关治疗提供借鉴。
Parkinson disease(PD)is one of the most common neurodegenerative diseases in the world and has become a research hotspot in recent years.The conventional dopamine replacement therapy in Western medicine has side effects,like motor fluctuations and visual hallucinations,making patients’compliance poor.PD belongs to the“tremor syndrome”in Chinese medicine.Guided by the theory of yin and fire,Professor Yu systematically summarises PD’s causative mechanism and treatment in different stages and proposes the core disease mechanism of burnt yin and fire.In the early stage of PD with insufficient true yin and yin fire,nourish kidney yin with modified Sancai Fengsui Dan;in the middle stage when there’s heart fire and yin fire,clear the heart and remove heat with modified Zuogui Wan;in the late stage,when there’s phlegm,phlegm stagnation,and gradually declining yin fire,resolve phlegm and blood stasis with modified Jinshui Liujun Decoction combined with Taohong Siwu Decoction.Based on the perspective of yin-fire derivation,this paper summarises the evolution of PD pathogenesis.It systematically summarises the treatment options for different stages of PD,to provide reference for the clinical treatment of PD.
