SPARCL1、Snail在胃癌组织中的表达及临床意义分析

摘要 目的观察富含半光氨酸的酸性分泌蛋白类蛋白1(SPARCL1)和Snail在胃癌中的表达及其临床意义。方法选取唐山市第三医院2018年1月~2023年1月手术切除的胃癌组织标本187例及癌旁非瘤组织标本56例。采用免疫组织化学法检测胃癌组织及癌旁组织中SPARCL1和Snail的表达。分析SPARCL1和Snail表达与病理特征的关系。比较行新辅助化疗和未行新辅助化疗患者术后标本中SPARCL1和Snail的表达情况,评估化疗对其表达的影响。结果SPARCL1在胃癌组织中的阳性率为28.34%,低于癌旁组织的83.92%,差异具有统计学意义(χ^(2)=54.983,P<0.001)。Snail在胃癌组织中的阳性率为77.01%,明显高于癌旁组织的19.64%,差异具有统计学意义(χ^(2)=61.387,P<0.001)。SPARCL1的表达水平与肿瘤分化程度、淋巴结转移及分期相关,Snail的表达水平与肿瘤浸润深度、淋巴结转移及分期相关。新辅助化疗组中SPARCL1高表达的比例为36.9%,高于未行新辅助化疗组的21.35%,差异具有统计学意义(χ^(2)=5.505,P=0.019)。两组间Snail的表达差异无统计学意义(P>0.05)。结论SPARCL1和Snail可以作为胃癌浸润转移的生物学标志物。化疗可能会影响胃癌患者SPARCL1的表型,但化疗是否导致表型转变仍需通过大样本和更严谨的研究验证。 Objective To observe the expression and clinical significance of SPARCL1 and Snail in gastric cancer.Methods A total of 187 cases of surgically resected gastric cancer tissue and 56 cases of adjacent non-tumor tissue specimens were collected from January 2018 to January 2023.Immunohistochemistry was used to detect the expression of SPARCL1 and Snail in gastric cancer and adjacent tissues.The relationship between the expression of SPARCL1,Snail,and pathological features was analyzed.Additionally,the expression of SPARCL1 and Snail in postoperative specimens from patients who received neoadjuvant chemotherapy was compared with those who did not,in order to evaluate the effect of chemotherapy on their expression.Results The positive rate of SPARCL1 in gastric cancer tissue was 28.34%,significantly lower than the 83.92%observed in adjacent tissues(χ^(2)=54.983,P<0.001).The positive rate of Snail in gastric cancer tissue was 77.01%,significantly higher than the 19.64%in adjacent tissues(χ^(2)=61.387,P<0.001).The expression level of SPARCL1 was correlated with tumor differentiation,lymph node metastasis,and staging,while the expression level of Snail was correlated with tumor invasion depth,lymph node metastasis,and staging.The proportion of high SPARCL1 expression in the neoadjuvant chemotherapy group was 36.9%,higher than the 21.35%in the non-neoadjuvant chemotherapy group(χ^(2)=5.505,P=0.019).No statistically significant difference in Snail expression was found between the two groups.Conclusion SPARCL1 and Snail can serve as biological markers for gastric cancer invasion and metastasis.Chemotherapy may influence the SPARCL1 phenotype in gastric cancer patients,but the potential phenotype conversion induced by chemotherapy requires further validation through large sample studies and more rigorous protocols.
