摘要
蒋筱教授认为,慢性荨麻疹(chronic urticaria,CU)发病的基础病机是脾虚生风,进而形成肝阳化风、血虚生风、津亏生风、瘀血生风、痰湿生风以及感受外风等六种不同病机。脾虚是CU发病的核心环节,由此诱发该病不同病机变化。蒋教授立足补土派“养正积自除”思想,提出CU的治疗应将健脾祛风法贯穿始终,同时兼顾疏肝、养血、润燥、活血、利湿、固表等,以阻截病机传变,自拟健脾祛风汤作为基础方,并根据不同患者病机特点与具体症状灵活加减。
Professor Jiang Xiao proposes that the fundamental pathogenesis of chronic urticaria(CU)lies in spleen deficiency generating wind,which further evolves into six distinct pathological patterns:liver yang transforming into wind,blood deficiency generating wind,fluid depletion generating wind,blood stasis generating wind,phlegm-turbidity generating wind,and exogenous wind invasion.Spleen deficiency is identified as the core mechanism triggering these pathological variations.Grounded in the Earth-Supplementing School′s principle of"nourishing the healthy qi to eliminate accumulation",Professor Jiang emphasizes that CU treatment should consistently integrate the spleen-fortifying and wind-eliminating method while synergistically addressing soothing the liver,nourishing blood,moistening dryness,activating blood,draining dampness,and consolidating the exterior.She has formulated Spleen-Fortifying and Wind-Eliminating Decoction as a foundational prescription,with tailored modifications based on individual pathological characteristics and clinical manifestations to intercept disease progression.
