摘要
目的对比腔内射频消融(RFA)联合泡沫硬化剂注射与点式抽剥术治疗下肢静脉曲张(VVLE)的临床效果。方法回顾性分析2021年3月至2023年7月濮阳市人民医院收治的101例VVLE患者,根据治疗方法不同分为联合组(RFA联合泡沫硬化剂注射,50例)、点式抽剥组(点式抽剥术,51例)。比较两组炎症指标[白介素-6(IL-6)、白介素-17(IL-17)、肿瘤坏死因子-α(TNF-α)]、血液流变学指标[血液黏度、纤维蛋白原、低切全血黏度]和阿伯丁静脉曲张问卷(AVVQ)评分。结果两组治疗总有效率差异无统计学意义(P>0.05)。术后3个月时,两组血清IL-6、IL-17、TNF-α水平均低于术前,且联合组低于点式抽剥组(P<0.05)。术后3个月时,两组血液黏度、纤维蛋白原、低切全血黏度水平、AVVQ评分均降低,且联合组低于点式抽剥组(P<0.05)。结论点式抽剥术与RFA联合泡沫硬化剂注射应用于VVLE治疗的疗效相当,但RFA联合泡沫硬化剂注射在改善血液流变学指标和炎症指标方面的效果更佳,有助于患者生活质量的改善。
Objective To compare the clinical effects of intracavitary rad iofrequency ablation(RFA)combined with injection of foam sclerosing agent with spot stripping in the treatment of vriose veins of lower extremity(VVLE).Methods A total of 101 patients with VVLE in the Puyang People’s Hospital from March 2021 to July 2023 were retrospectively analyzed.According to the different treatment methods,they were divided into the combined group(RFA combined with foam sclerosing agent injection,50 cases)and the spot stripping group(spot stripping,51 cases).The curative effect,inflammatory indexes[interleukin-6(IL-6),interleukin-17(IL-17),tumor necrosis factor-α(TNF-α)],hemorheology indexes[blood viscosity,fibrinogen,low shear whole blood viscosity]and Aberdeen varicose vein questionnaire(AVVQ)score were evaluated before and 3 months after operation.Results There was no statistical significant difference in the total effective rate between the two treatment groups(P>0.05).At 3 months after operation,the levels of IL-6,IL-17 and TNF-αin the two groups were lower than those before operation,and those in the combined group were lower than the point stripping group(P<0.05).At 3 months after operation,the levels of blood viscosity,fibrinogen and low-cut whole blood viscosity in the two groups decreased,and those in the combined group were lower than the point stripping group(P<0.05).Compared to before treatment,the AVVQ scores of the two groups decreased,and the combined group was lower than the spot stripping group(P<0.05).Conclusion The efficacy of spot stripping is equivalent to that of RFA combined with foam sclerosing agent injection in the treatment of VVLE,but RFA combined with foam sclerosing agent injection is more effective in improving hemorheological indicators and inflammatory indicators,helping to improve the quality of life of patients.
