摘要
目的:分析全民健康体检中超声诊断对乳腺病变的检出价值,并分析其相关对策。方法:收集2023年1月~12月在本院体检中心进行乳腺超声筛查的15985例女性相关资料,对于BI-RADS≥4类的检查者均建议病理学检查,以病理学检查结果为依据,分析乳腺超声对乳腺病变性质的诊断效能。结果:经乳腺超声检查,15985例检查者中,有5113例检查出乳腺病变,占31.99%,其中乳腺增生占比最高,为20.46%,乳腺癌检出率为0.22%;15985例检查者中,未检出乳腺病变10872例,占68.01%。15985例检查者中,经乳腺超声检查有64例BI-RADS≥4类,占0.40%。64例BI-RADS≥4类患者均行病理学检查,结果显示,39例确诊为乳腺癌,25例明确为乳腺良性病变。以病理学检查结果为依据,乳腺超声筛查乳腺恶性病变的灵敏度为87.18%、特异度为96.00%、准确度为90.63%、阳性预测值97.14%、阴性预测值82.76%;乳腺超声筛查乳腺恶性病变与病理学结果具有极好的一致性(Kappa=0.809,P<0.05)。结论:全民健康体检中超声检查可有效检出乳腺病变,且对诊断乳腺恶性病变的价值较高;建议今后临床通过超声筛查乳腺病变,以提高病变检出率。
Objective:To analyze the value of ultrasound diagnosis in detecting breast lesions during national health check ups,and to analyze its related strategies.Methods:Collect relevant data from 15985 women who underwent breast ultrasound screening at our hospital’s physical examination center from January to December 2023.For those with BI-RADS≥4 categories,pathological examination is recommended.Based on the pathological examination results,analyze the diagnostic efficacy of breast ultrasound for the nature of breast lesions.Results:Among 15985 examinees,5113(31.99%)had breast lesions detected by breast ultrasound,of which 20.46%were breast hyperplasia,and 0.22%were breast cancer;Among 15985 examinees,10872(68.01%)did not detect breast lesions.Among 15985 examinees,64 cases(0.40%)had BI-RADS≥4 categories detected by breast ultrasound examination.64 patients with BI-RADS≥4 types underwent pathological examination.The results showed that 39 cases were diagnosed as breast cancer and 25 cases were confirmed as benign breast lesions.Based on pathological examination results,the sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of breast ultrasound screening for malignant breast lesions were 87.18%,96.00%,90.63%,97.14%,and 82.76%,respectively;Breast ultrasound screening for malignant breast lesions showed excellent consistency with pathological results(Kappa=0.809,P<0.05).Conclusion:Ultrasound examination in national health checkups can effectively detect breast lesions and has high value in diagnosing malignant breast lesions;It is recommended to use ultrasound screening for breast lesions in clinical practice in the future to improve the detection rate of lesions.
