摘要
目的:探究全自动凝血分析仪在自动检测凝血酶时间时的失败原因,并制定出相应的解决措施。方法:选取2023年6月~2023年12月本院住院部5063份TT检测样本作为研究对象,其中检测失败样本为222份,研究分析失败样本的原因及制定针对性的应对措施。结果:在所收集的5063份TT检测样本中,失败样本为222份(4.38%)。脂血样本91例(41.00%)、肝素干扰样本52例(23.40%)、患者使用口服抗凝药样本50例(22.50%)和29例(13.10%)样本存在微小凝块或血浆量不足;经仪器报警信息分析表明,在所研究的222份失败样本中,基线期吸光度变化值增大、二阶导数无峰值样本和反应过程吸光度变化值<35mAbs样本、起始点过低及无法找到起始点样本、无凝固曲线样本分别为72例(32.40%)、68例(30.60%)、57例(25.70%)、19例(8.60%)和6例(2.70%)。结论:在分析失败样本原因时,详细阐述出脂血样本、肝素干扰样本、患者使用口服抗凝药样本等样本运用全自动凝血分析仪凝血酶时间自动检测失败的具体原因,同时对全自动凝血分析仪报警信息进行深入分析,进而为随后制定出应对措施提供重要的参考依据。
Objective:To explore the reasons for failure of automatic coagulation analyzer when detecting thrombin time automatically,and to work out corresponding solutions.Methods:A total of 5063 TT test samples from the inpatient department of our hospital from June 2023 to December 2023 were selected as the research objects,among which 222 failed test samples were analyzed to study the causes of failure samples and develop targeted countermeasures.Results:Among the 5063 TT test samples collected,222(4.38%)failed.There were micro clots or insufficient plasma volume in 91(41.00%)lipid samples,52(23.40%)heparin interference samples,50(22.50%)and 29(13.10%)oral anticoagulants.The analysis of instrument alarm information shows that among the 222 failed samples studied,there were 72 cases(32.40%),68 cases(30.60%)and 57 cases(25.7%),19 cases(8.60%)and 6 cases(2.70%)with the increase of the absorbance change value in the baseline period,the samples with no peak value in the second derivative and the samples with less than 35mAbs during the reaction process,and the samples with too low starting point and no solidification curve,respectively.Conclusion:In the analysis of the reasons for the failure of samples,the specific reasons for the failure of automatic thrombin time detection by automatic coagulation analyzer were described in detail,such as lipid blood samples,heparin interference samples,and oral anticoagulant samples used by patients.Meanwhile,the alarm information of automatic coagulation analyzer was deeply analyzed,thus providing an important reference basis for the subsequent formulation of countermeasures.
