摘要
痛风是一种常见的慢性风湿性疾病,长期血尿酸升高会导致关节、组织中尿酸钠晶体沉积,从而引起关节疼痛和功能受损,给患者的正常生活带来困扰,严重者可累及血管、内脏,甚至直接危及生命。随着人们生活方式和饮食结构的改变,痛风的患病率呈上升趋势,且越来越年轻化。因此许多临床研究愈加关注痛风的发病与治疗。近年来,中医药的发展有目共睹,在痛风治疗中效果显著。中药单味药、常用药对及复方在痛风治疗中的应用十分常见,这使得我们更加迫切地想要了解这些药物通过何种机制作用于人体,从而减轻患者的痛苦、改善相关机体功能。网络药理学方法使得我们揭开了中医药治疗痛风的神秘面纱,研究显示这些药物能够很好地作用于痛风疾病网络中的多个靶点和信号通路,在控制尿酸生成、降低血清尿酸含量、抑制炎症因子表达、改善疾病预后等多个方面发挥重要作用,能够有效减轻临床症状、减少发作频率。笔者对数十余篇相关文献进行阅读和整理,分别从中药单味药、常用药对及中药复方治疗痛风的有效成分、作用靶点、作用通路等方面进行综述,虽不能涵盖全局,但希望为各位同道临床用药提供参考。
Gout is a common chronic rheumatic disease.Long-term increase in blood uric acid will lead to the deposit of sodium urate crystals in joints and tissues,which will cause joint pain and function impairment,and bring troubles to patients’normal life.In serious cases,blood vessels and internal organs can be affected,and even directly endanger life.With the change of people’s lifestyle and diet structure,the prevalence of gout is on the rise,and the patients are getting younger and younger.Therefore,many clinical studies have increasingly focused on the pathogenesis and treatment of gout.In recent years,the development of traditional Chinese medicine has been widely recognized,and we have found that it is effective in the treatment of gout.The application of traditional Chinese medicine in the treatment of gout is very common,which makes us more urgent to understand the mechanism of drugs on the human body,so as to play an effective role in alleviating patients’pain and improving related body functions.Network pharmacology enables us to uncover the mystery of traditional Chinese medicine in the treatment of gout.Studies have shown that these drugs can effectively act on multiple targets and signaling pathways in the gout disease network,and play an important role in controlling uric acid production,reducing serum uric acid content,inhibiting the expression of inflammatory factors,and ameliorating disease prognosis.It can effectively reduce clinical symptoms and reduce the frequency of attacks.In this paper,more than dozens of relevant studies are read and sorted out,and the effective ingredients,action targets,action pathways and other aspects of traditional Chinese medicine in the treatment of gout are reviewed respectively.Although it could not cover all aspects,it is hoped that it could provide references for the clinical drug use of fellow practitioners.
