摘要
目的了解河南省中医医院门诊血液透析患者的血液透析事件发生情况。方法采用前瞻性的调查方法,基于河南省3家代表性的中医医院,选择2022年1—12月每月前2个工作日内维持血液透析治疗的3486例门诊患者为研究对象,基于美国疾病预防控制中心国家医疗保健安全网自行设计血液透析事件监测表格观察门诊患者血液透析事件情况。结果监测血液透析事件发生率为2.50%(87/3486),其中最高为非隧道式中心静脉置管[3.84%(17/443)],最低为内瘘置管[1.34%(21/1562)]。全身使用抗菌药物感染患者中,血管通路相关感染占比高达61.11%(22/36);血管穿刺部位感染以非隧道式中心静脉导管最高,为2.30%(9/443)。随着监测进行,血液透析事件发生率总体呈下降趋势。结论河南省中医医院门诊血液透析患者血管穿刺部位感染较为常见,长期连续监测有助于早期发现感染的风险及问题,及时采取针对性的预防控制措施,减少血液透析事件的发生。
Objective To know the current status of hemodialysis events in the hospitals of Chinese medicine from Henan province.Methods A total of 3486 patients who underwent hemodialysis within the first two workdays of every month for outpatient hemodialysis were selected in hemodialysis center of three representative hospitals of Chinese medicine.Self-designed questionnaire from National Healthcare Safety Network was used to collect hemodialysis events of hemodialysis patients from January to December 2022.Results Hemodialysis events with the incidence rate of 2.50%(87/3486).The highest incidence rate of hemodialysis events was non-tunnel central venous catheterization[3.84%(17/443)],the lowest was the placement of an internal fistula catheter[1.34%(21/1562)].The blood vessel puncture site infections up to 61.11%(22/36)among the systemic use of antibiotics patients.The highest blood vessel puncture site infections among different catheterization was 2.30%(9/443)from non-tunnel central venous catheterization.The incidence rate of hemodialysis events was decreased with month.Conclusion The blood vessel puncture site infections are common in the hemodialysis outpatient.It is helpful to take targeted prevention measures base on infection risk and reduce the incidence of hemodialysis events through the long-term continuously surveillance.
