摘要
目的:探讨桂枝茯苓丸联合屈螺酮炔雌醇治疗肥胖型多囊卵巢综合征(obese polycystic ovary syndrome,PCOS)的效果。方法:选取2020年6月—2023年1月莆田市第一医院妇产科收治的98例PCOS患者作为研究对象,按随机数表法分为对照组(n=49)和研究组(n=49)。对照组在对症治疗基础上给予屈螺酮炔雌醇治疗,研究组在对照组基础上给予桂枝茯苓丸治疗,对比两组干预前后性激素[促黄体生成素(luteinizing hormone,LH)、促卵泡生成素(follicle-stimulating hormone,FSH)、雌激素(estrogen,E_(2))、睾酮(testosterone,T)]、胰岛素抵抗指标[胰岛素抵抗指数(homeostatic model assessment of insulin resistance,HOMA-IR)、空腹胰岛素(fasting Insulin,FINS)]及干预后排卵率和妊娠率。结果:干预后,两组LH、FSH、E_(2)、T、FINS、HOMA-IR水平降低,且研究组较对照组更低,差异有统计学意义(P<0.05);研究组排卵率、妊娠率较对照组更高,差异有统计学意义(P<0.05)。结论:枝茯苓丸联合屈螺酮炔雌醇治疗肥胖型PCOS患者可改善性激素水平,调节胰岛素抵抗,促进排卵,提高妊娠率。
Objective:To explore the effect of Guizhi Fuling Pills combined with Drospirenone Ethinylestradiol in the treatment of obese polycystic ovary syndrome(PCOS).Method:A total of 98 patients with obese PCOS who admitted to the Obstetrics and Gynecology Department of Putian First Hospital from June 2020 to January 2023 were selected as the study subjects,they were randomly divided into the control group(n=49)and the study group(n=49)using the random number table method.The control group was treated with Drospirenone Ethinylestradiol on the basis of symptomatic treatment,and the study group was treated with Guizhi Fuling Pills on the basis of the control group,the sex hormones[luteinizing hormone(LH),follicle-stimulating hormone(FSH),estrogen(E_(2)),testosterone(T)],insulin resistance index[homeostatic model assessment of insulin resistance(HOMA-IR),fasting insulin(FINS)]before and after intervention,and ovulation rate and pregnancy rate after intervention were compared between two groups.Result:After intervention,the levels of LH,FSH,E_(2),T,FINS and HOMA-IR in two groups were decreased,and those in the study group were lower than those in the control group,the differences were statistically significant(P<0.05);the ovulation rate and pregnancy rate in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Guizhi Fuling Pills combined with Drospirenone Ethinylestradiol in the treatment of obese PCOS can improve sex hormone levels,regulate insulin resistance,promote ovulation,and increase pregnancy rate.
