摘要
文章总结张士卿教授分期辨治小儿厌食症的临证经验。张教授师承古训,结合当今社会儿童膳食情况,认为小儿厌食症病机关键是脾失健运、食滞湿阻,临证主张虚实辨证,治疗秉承“脾健不在补贵在运”学术思想,重视小儿生长发育的生理特点,提出分三期治疗:初期以攻为主,消食导滞祛食湿;中期攻补兼施,运脾消积开胃口;后期以补为主,健脾益肾助生长。遣方用药“经方头,时方尾”,临床疗效显著。
This article has summarized the clinical experience of Professor Zhang Shiqing in the staged differentiation and treatment of infantile anorexia(IA).Professor Zhang,who has inherited the ancient teachings and taken into consideration children's dietary conditions in today's society,believes that key to the pathogenesis of IA is the inability of the spleen to transport and transform food,food stagnation and dampness obstruction.Therefore,it is necessary to distinguish between deficiency and excess of the disease first,and in treatment follow the principle of transporting but not tonifying to invigorate the spleen,also pay due attention to the growth and development of children.He proposes three stages of treatment for IA.In the early stage,purging is dominated to dissipate food retention and dispel food dampness.In the middle stage,purging and tonifying should be combined to transport the spleen,eliminate food retention and improve appetite.In the last stage,tonifying is dominated to invigorate the spleen and tonify the kidneys to promote growth.When applying prescriptions,“classical prescriptions come first,followed by modified prescriptions”,which has shown confirmed clinical efficacy.
