摘要
目的本研究通过随机对照试验,明确评估参芪抑毒养肺汤在西医治疗基础上对病毒性重症肺炎患者的治疗效果。方法选取2020年1月1日—2023年2月26日于某医院就诊的病毒型重症肺炎患者84例,按照随机数表法分成西医治疗组和中西医结合治疗组,各42例。采用放射免疫分析法检测血清IL-6、IL-8、IL-2受体、TNF-α和超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)含量;流式细胞术测定外周血淋巴细胞亚群CD4^(+)T淋巴细胞计数、CD8^(+)T淋巴细胞计数;使用全自动血液流变仪测量白细胞(WBC)计数、中性粒细胞(neutrophil,N)计数和淋巴细胞(lymphocyte,L)计数。结果中西医结合治疗组IL-6和TNF-α水平低于西医治疗组,差异有统计学意义(t=-2.486、2.187,P<0.05);两组患者的CD4^(+)T淋巴细胞和CD8^(+)T淋巴细胞计数,差异无统计学意义(t=0.344、0.462,P>0.05);两组患者中WBC(治疗前WBC、治疗后WBC、δ-WBC)计数、L(治疗前L、治疗后L、δ-L)计数、IL-8、IL-2R、hs-CRP水平比较,差异无统计学意义(t=-1.138、-0.945、-1.652、-0.542、-0.012、0、0、0.832、1.023,P>0.05)。结论参芪抑毒养肺汤可降低病毒性重症肺炎患者体内炎症因子水平,提高整体治疗效果。
Objective To clearly assess the therapeutic effect of Shenqi Yidou Yangfei decoction on patients with severe viral pneumonia based on western medical treatment through a randomized controlled trial in this study.Methods A total of 84 patients with severe viral pneumonia who visited a hospital from January 1,2020 to February 26,2023 were selected and divided into two groups of the western medicine treatment group and the integrated traditional Chinese and western medicine treatment group according to the random number table method,each with 42 cases.The contents of IL-6,IL-8,IL-2 receptor,TNF-α,and hypersensitive C-reactive protein(hs-CRP)in serum were detected using the radioimmunoassay method.The count of CD4^(+)T lymphocytes and CD8^(+)T lymphocytes in lymphocyte subsets of peripheral blood were measured by flow cytometry.The count of white blood cells(WBC),neutrophils(N),and lymphocytes(L)were measured using a fully automatic hematology analyzer.Results The levels of IL-6 and TNF-αin the integrated traditional Chinese and western medicine treatment group were lower than those in the western medicine treatment group,and the difference was statistically significant(t=-2.486,2.187,P<0.05).The difference in the count of CD4^(+)T lymphocytes and CD8^(+)T lymphocytes in the two groups of patients was not statistically significant(t=0.344,0.462,P>0.05).There were no statistically significant differences in WBC(pre-treatment WBC,post-treatment WBC,δ-WBC)count,L(pre-treatment L,post-treatment L,δ-L)count,the levels of IL-8,IL-2R,and hs-CRP compared among the two groups of patients(t=-1.138,-0.945,-1.652,-0.542,-0.012,0,0,0.832,1.023,P>0.05).Conclusion Shenqi Yidu Yangfei decoction can reduce the levels of inflammatory factors in patients with severe viral pneumonia and improved the overall therapeutic effect.
