摘要
目的探究青光眼患者自我管理行为的内在体验,为构建针对该群体自我管理干预方案提供依据。方法采用现象学研究方法,通过目的性抽样对16例青光眼患者进行一对一半结构式访谈。利用Colaizzi的7步分析法对访谈数据进行主题提炼。结果青光眼患者自我管理行为内在体验可归纳为:自我管理认知不足,患者普遍缺乏对疾病的认识,包括对疾病知识的缺乏、对症状的不合理归因以及对医疗权威的过度依赖;自我管理困扰体验,包括睡眠障碍、日常活动受限以及对未来失明的恐惧;自我管理行为的调节与适应,包括积极面对、重视健康管理、渴望支持。结论青光眼患者自我管理存在诸多障碍,医务人员应通过多途径解决患者自我管理阻碍因素,提高患者眼压控制能力,促进视力恢复,并提升视觉相关生活质量。
Objective To explore the intrinsic experiences of glaucoma patients in self-management behaviors,and to provide a theoretical basis for constructing self-management intervention programs for this group.Methods Phenomenological research methods were employed,with purposeful sampling of 16 glaucoma patients for one-on-one semi-structured interviews.Colaizzi's 7-step analysis method was used to extract themes from the interview data.Results The intrinsic experiences of glaucoma patients in self-management behaviors can be summarized into three main categories:(1)Insufficient self-management cognition,where patients generally lack awareness of the disease,including a lack of disease knowledge,irrational attribution of symptoms,and excessive dependence on medical authority(power distance).(2)Experiences of self-management difficulties,including sleep disorders,limitations in daily activities,and fear of future blindness.(3)Regulation and adaptation of self-management behaviors,including active confrontation,emphasis on health management,and a desire for support.Conclusion There are many obstacles in the self-management of glaucoma patients.Medical personnel should address the factors hindering patient self-management through multiple approaches,improve patients'intraocular pressure control ability,promote the recovery of vision,and enhance the quality of life related to vision.
