一站式全脑CT灌注成像检查定量参数对动脉瘤性蛛网膜下腔出血的评估价值

摘要 目的 探讨一站式全脑CT灌注成像(CTP)检查对动脉瘤性蛛网膜下腔出血(aSAH)的评估价值。方法 回顾性分析我院2020年12月至2022年12月收治的88例aSAH患者的临床资料,对比不同Hunt-Hess分级、Fisher分级患者的CTP定量参数差异。采用Spearman相关性分析CTP定量参数与Hunt-Hess分级、Fisher分级的相关性。结果 Hunt-HessⅠ级、Ⅱ级、Ⅲ级患者的CTP定量参数脑血流量(CBF),脑血容量(CBV),平均通过时间(MTT),达峰时间(TTP)差异有统计学意义(P<0.05),Ⅰ级患者的CBF、CBV>Ⅱ级患者>Ⅲ级患者,Ⅰ级患者的MTT、TTP<Ⅱ级患者<Ⅲ级患者,不同Hunt-Hess分级患者的CTP定量参数比较有统计学意义(P<0.05);Fisher 2级、3级、4级患者的CBF,CBV,MTT,TTP差异有统计学意义(P<0.05),2级患者的CBF、CBV>3级患者>4级患者,2级患者的MTT、TTP<3级患者<4级患者,差异有统计学意义(P<0.05),不同Fisher分级患者的CTP定量参数比较差异有统计学意义(P<0.05)。Spearman相关性分析显示,CBF及CBV与Hunt-Hess分级、Fisher分级均呈负相关(P<0.05);平均通过时间(MTT、)及达峰时间(TTP)与Hunt-Hess分级、Fisher分级均呈正相关(P<0.05)。结论 一站式全脑CTP定量参数CBF、CBV、MTT及TTP与aSAH病情严重程度有明显相关性,一站式全脑CTP检查在aSAH患者的病情评估中有重要价值,CBF、CBV、MTT及TTP等CTP定量参数与aSAH病情严重程度密切相关,可用于病情评估。 Objective To explore the relationship between quantitative parameters of one-stop whole brain CT perfusion imaging(CTP)examination and severity of aneurysmal subarachnoid hemorrhage(aSAH).Methods The clinical data of 88 patients with aSAH admitted to the hospital from December 2020 to December 2022 were retrospectively analyzed,and the differences in CTP quantitative parameters were compared among patients with different Hunt-Hess grades and Fisher grades,and Spearman correlation analysis was used to analyze the correlation between CTP quantitative parameters and Hunt-Hess grading and Fisher grading.Results The CTP quantitative parameter cerebral blood flow(CBF)values in patients with Hunt-Hess gradesⅠ,ⅡandⅢ,the cerebral blood volume(CBV)values the mean transit time(MTT)values and the time to peak(TTP)values had significant differences.The CBF and CBV were manifested as gradeⅠpatients>gradeⅡpatients>gradeⅢpatients,while the MTT and TTP revealed gradeⅠpatients<gradeⅡpatients<gradeⅢpatients(P<0.05).The CBF values in patients with Fisher grades 2,3 and 4,CBV values,MTT values,and TTP values had significant differences(P<0.05),and CBF and CBV showed grade 2 patients>grade 3 patients>grade 4 patients while MTT and TTP were manifested as grade 2 patients<grade 3 patients<grade 4 patients(P<0.05).Spearman correlation analysis showed that CBF and CBV were negatively correlated with Hunt-Hess grading and Fisher grading(P<0.05).MTT and TTP were positively correlated with Hunt-Hess grading and Fisher grading(P<0.05).Conclusion One-stop whole brain CTP quantitative parameters of CBF,CBV,MTT and TTP are significantly correlated with the severity of aSAH.
